自拟益气补血膏治疗妊娠期缺铁性贫血的效果及对妊娠结局的影响

Effect of Self-prepared Yiqi Buxue Paste on Iron Deficiency Anemia during Pregnancy and Its Impact on Pregnancy Outcome

目的探讨自拟益气补血膏联合蛋白琥珀酸铁口服溶液治疗妊娠期缺铁性贫血(IDA)的效果及对血常规、铁代谢指标、安全性、妊娠结局的影响。方法选取2019年1月—2021年8月收治的妊娠期IDA 70例,按照治疗方案分为观察组和对照组各35例。对照组给予蛋白琥珀酸铁口服溶液治疗,观察组在对照组基础上给予自拟益气补血膏治疗。比较2组临床疗效,检测治疗前及治疗2、4周后血常规指标[血红蛋白(Hb)、红细胞(RBC)、平均红细胞体积(MCV)、平均血红蛋白浓度(MCHC)]、铁代谢指标[血清铁(SI)、铁蛋白(SF)、铁幼素(HJV)、铁调素-25(Hepcidin-25)]、血清学指标[γ干扰素(IFN-γ)、白细胞介素-2(IL-2)、氧磷酶1(PON1)]水平,并比较2组用药安全性及妊娠结局。结果观察组总有效率高于对照组(P<0.05)。观察组治疗2、4周后Hb、RBC、MCV、MCHC、SI、SF、HJV、Hepcidin-25、IFN-γ、IL-2、PON1水平均高于对照组(P<0.01)。2组不良反应发生率比较差异无统计学意义(P>0.05)。观察组不良妊娠结局发生率低于对照组(P<0.05)。结论自拟益气补血膏联合蛋白琥珀酸铁口服溶液治疗妊娠期IDA临床效果确切且安全性较高,可改善贫血状态,促进铁代谢,减少不良妊娠结局。

Objective To investigate the effect of Yiqi Buxue Paste combined with oral solution of protein iron succinate in the treatment of iron deficiency anemia(IDA)during pregnancy and its impact on blood routine,iron metabolism indexes,safety and pregnancy outcome.Methods Seventy patients with IDA during pregnancy admitted from January 2019 to August 2021 were selected and divided into observation group(n=35)and control group(n=35)according to treatment plan.The control group was treated with oral solution of iron protein succinate,and the observation group was treated with self-prepared Yiqi Buxue Paste on the basis of the control group.The clinical efficacy,blood routine indexes[hemoglobin(Hb),red blood cells(RBC),mean red blood cell volume(MCV),mean hemoglobin concentration(MCHC)],iron metabolism indexes[serum iron(SI),ferritin(SF),ferritin(HJV),ferritonin-25(Hepcidin-25)]and serological indexes[γinterferon(IFN-γ),interleukin-2(IL-2),and paraoxonase 1(PON1)]before treatment and at 2 and 4 weeks after treatment were compared between the two groups.The drug safety and pregnancy outcome were compared between the two groups.Results The total effective rate of observation group was higher than that of control group(P<0.05).At 2 and 4 weeks after treatment,the levels of Hb,RBC,MCV,MCHC,SI,SF,HJV,Hepcidin-25,IFN-,IL-2,and PON1 in observation group were higher than those in control group(P<0.01).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).The incidence of adverse pregnancy outcome in observation group was lower than that in control group(P<0.05).Conclusion The self-prepared Yiqi Buxue Paste combined with oral solution of protein iron succinate in the treatment of IDA during pregnancy has definite clinical effect and good safety,which can improve anemia,promote iron metabolism and reduce adverse pregnancy outcomes.
