阿立哌唑治疗奥氮平引起女性精神分裂症代谢综合征的临床效果

Clinical effect of aripiprazole on metabolic syndrome in female schizophrenic patients caused by olanzapine

目的分析女性精神分裂症患者情况,观察阿立哌唑治疗奥氮平引起女性精神分裂症代谢综合征患者的临床效果。方法选取2019年12月—2020年1月临沂市精神卫生中心收治的奥氮平引起女性精神分裂症代谢综合征患者80例,根据随机数字表法分为观察组和对照组,每组40例。对照组采用利培酮治疗,观察组采用阿立哌唑治疗,2组均治疗4周。比较2组患者治疗总有效率,治疗前后精神症状评分、生活质量评分、代谢相关指标变化及不良反应。结果观察组治疗总有效率为92.50%,高于对照组的75.00%,差异有统计学意义(χ^(2)=4.500,P=0.033);治疗4周后,2组阳性与阴性症状量表(PANSS)评分较治疗前均下降,且观察组低于对照组(P均<0.01);2组腰围、体质指数(BMI)较治疗前减小,观察组空腹血糖、三酰甘油水平明显低于治疗前,且观察组治疗后腰围、BMI小于对照组,空腹血糖、三酰甘油水平低于对照组(P均<0.01);2组物质生活、躯体功能、心理功能、社会功能评分较治疗前均升高,且观察组各项评分高于对照组(P均<0.01);观察组与对照组不良反应总发生率比较差异无统计学意义(15.00%vs.12.50%,χ^(2)=0.105,P=0.745)。结论阿立哌唑有助于改善奥氮平引起女性精神分裂症代谢综合征情况,进而促进患者精神症状改善,治疗效果显著,提升患者生活质量。

Objective To analyze the situation of female schizophrenic patients and observe the clinical effect of aripiprazole on metabolic syndrome in female schizophrenic patients caused by olanzapine.Methods 80 female schizophrenic patients with metabolic syndrome caused by olanzapine in Linyi Mental Health Center from December 2019 to January 2020 were selected.The control group and the observation group were randomly divided into 40 cases each.The control group was treated with risperidone and the observation group was treated with aripiprazole.Both groups were treated continuously for 4 weeks.Compare the total effective rate of the two groups of patients,the scores of mental symptoms,quality of life,changes of metabolic related indicators before and after treatment,and the adverse reactions of the two groups.Results The total effective rate of the observation group was 92.50%,higher than 75.00%in the control group(χ^(2)=4.500,P=0.033);After 4 weeks of treatment,the scores of positive and negative symptom scale(PANSS)in both groups decreased compared with those before treatment,and the scores in the observation group were lower than those in the control group(P<0.01);The waist circumference and body mass index(BMI)of the two groups were lower than those before treatment,and the fasting blood glucose and triacylglycerol levels in the observation group were significantly lower than those before treatment,and those in the observation group were lower than those in the control group after treatment(all P<0.01);The scores of material life,physical function,psychological function and social function in the two groups were higher than those before treatment,and the scores in the observation group were higher than those in the control group(all P<0.01);The total incidence of adverse reactions in the observation group was 15.00%,while that in the control group was 12.50%,with no significant difference(χ^(2)=0.105,P=0.745).Conclusion Aripiprazole treatment can improve the metabolic syndrome of female schizophrenic patients caused by olanzapine,and then promote the improvement of mental symptoms.The treatment effect is significant and improve the quality of life of patients.
