健脾和胃汤辅治慢性浅表性胃炎多并症的临床效果

Clinical effect of Jianpi Hewei decoction on chronic superficial gastritis

目的观察慢性浅表性胃炎多并症患者采取健脾和胃汤治疗的临床效果。方法选取2017年9月—2020年8月福建中医药大学附属三明中西医结合医院/三明市中西医结合医院收治的慢性浅表性胃炎多并症患者80例,根据随机数字表法分为中药联合组和常规用药组,每组40例。中药联合组在常规抗感染治疗基础上联合健脾和胃汤治疗,常规用药组在常规抗感染治疗基础上予多潘立酮片、雷贝拉唑治疗,2组患者均治疗2周。比较2组治疗效果、临床症状改善时间、住院时间,治疗前后炎性指标、急性生理与慢性健康评分(APACHE-Ⅱ)变化及不良反应。结果中药联合组治疗总有效率为97.50%,高于常规用药组的77.50%(χ^(2)=7.314,P=0.007);中药联合组胃部疼痛改善时间、食欲减退恢复时间与住院时间均短于常规用药组(P<0.01);治疗2周后,2组白介素-6(IL-6)、C反应蛋白(CRP)、降钙素原(PCT)、D-二聚体(D-D)水平较治疗前均降低,且中药联合组低于常规用药组(P均<0.01);2组APACHE-Ⅱ评分较治疗前均下降,且中药联合组低于常规用药组(P均<0.01);中药联合组不良反应总发生率为2.50%,低于常规用药组的25.00%(χ^(2)=8.538,P=0.003)。结论慢性浅表性胃炎多并症患者在常规抗感染治疗基础上联合健脾和胃汤治疗效果佳,有助于改善病情,缩短胃部疼痛改善时间,降低患者不适感,减少不良反应,安全性高,可临床推广应用。

Objective To observe the clinical effect of Jianpi Hewei decoction on chronic superficial gastritis.Methods From September 2017 to August 2020,80 patients with multiple complications of chronic superficial gastritis were selected from Sanming Integrated Traditional Chinese and Western Medicine Hospital Affiliated to Fujian University of Traditional Chinese Medicine/Sanming Integrated Traditional Chinese and Western Medicine Hospital.They were divided into herb joint group and general group according to random number table,with 40 patients in each group.The herb joint group was treated with Jianpi Hewei decoction on the basis of conventional anti infection treatment,and the general group was treated with domperidone tablets and rabeprazole on the basis of conventional anti infection treatment.Both groups were treated for 2 weeks.The therapeutic effect,improvement time of clinical symptoms,hospitalization time,changes of inflammatory indexes,acute physiology and chronic health score(APACHE-Ⅱ)before and after treatment,and adverse reactions were compared between the two groups.Results The total effective rate of the herb joint group was 97.50%,higher than 77.50%in the general group(χ^(2)=7.314,P=0.007);The improvement time of gastric pain,the recovery time of anorexia and the hospitalization time in the herb joint group were shorter than those in the general group(P<0.01);After 2 weeks of treatment,the levels of interleukin-6(IL-6),C-reactive protein(CRP),procalcitonin(PCT)and D-dimer(D-D)in the two groups were lower than those before treatment,and the levels in the herb joint group were lower than those in the general group(all P<0.01);The APACHE-Ⅱscores of the two groups decreased compared with that before treatment,and that of the herb joint group was lower than that of the general group(all P<0.01);The total incidence of adverse reactions in the herb joint group was 2.50%,lower than 25.00%in the control group(χ^(2)=8.538,P=0.003).Conclusion Chronic superficial gastritis patients are treated with Jianpi Hewei decoction,the treatment effect is good,which is helpful for the improvement of the patient′s condition,shortens the improvement time of stomach pain,reduces the patient′s discomfort,and reduces the occurrence of adverse reactions.It is safe and can be clinical promotion.
