自拟皂荚汤在慢性阻塞性肺疾病戒烟后肺功能的临床观察

Clinical effect of self-prepared Gleditsia sinensis decoction on lung function of patients with chronic obstructive pulmonary disease after smoking cessation

目的 探究自拟皂荚汤在中老年戒烟者慢阻肺戒烟的临床疗效。方法 选取2018年5月—2021年5月在郑州大学第五附属医院诊治的40例中老年慢阻肺戒烟患者,随机分为对照组(20例)和观察组(20例),对照组给予心理干预和健康宣教治疗,观察组在对照组的基础上给予自拟皂荚汤治疗,治疗前及治疗2个月后对两组患者肺活量(VC)、最大呼气流量(PEF)、用力肺活量(FVC)、6 min步行距离(6 MWD)、慢阻肺评估问卷(CAT)进行观察。结果 治疗后两组组间、组内比较患者肺功能指标较治疗前升高(P<0.05),组间比较观察组升高更明显(P<0.05);两组患者CAT评分均较前下降(P<0.05),观察组下降更明显(P<0.05);两组治疗后6MWD增加较治疗前明显(P<0.05),观察组增加更明显(P<0.05)。结论 自拟皂荚汤可明显改善中老年COPD患者戒烟后肺功能,可提高患者的日常生活能力及运动耐力。

Objective To explore the clinical effect of self-prepared Gleditsia sinensis decoction on lung function of patients with chronic obstructive pulmonary disease. Method Forty middle-aged and elderly patients with COPD who gave up smoking from May 2018 to May 2021 were selected and randomly divided into control group(20 cases) and observation group(20 cases). The control group was given psychological intervention and health education treatment, and the observation group was given self-prepared gleditsia decoction on the basis of the control group. The vital capacity(VC), maximum expiratory flow(PEF), forced vital capacity(FVC), 6-min walking distance(6 MWD) and COPD assessment questionnaire(CAT) of the two groups were observed before and 2 months after treatment. Results After treatment, the pulmonary function indexes in the two groups were higher than before treatment(P<0.05), and the increase was more obvious in the observation group(P<0.05). CAT scores in 2 groups were decreased compared with before(P<0.05), and the decrease was more obvious in the observation group(P<0.05). The increase of 6MWD in the two groups after treatment was significantly higher than that before treatment(P<0.05), and the increase was more significant in the observation group(P<0.05). Conclusion Self-prepared Gleditsia sinensis decoction can significantly improve lung function, daily living ability and exercise endurance of middle-aged and elderly COPD patients after smoking cessation.
