联合检测RBP、HBA1c、CysC、ACR对2型糖尿病早期肾损伤的诊断价值 被引量:1

Diagnostic value of combined detection of RBP,HBA1c,CysC and ACR in early renal injury of type 2 diabetes

摘要 目的探讨联合检测视黄醇结合蛋白(RBP)、糖化血红蛋白(HBA1c)、胱抑素C(CysC)、尿微量白蛋白与肌酐比值(ACR)对2型糖尿病(T2DM)早期肾损伤的诊断价值。方法回顾性分析,收集2021年1月-2022年1月都昌县人民医院收治的T2DM早期肾损伤患者40例临床资料,作为观察组,并收集同期该院收治的T2DM患者40例临床资料,作为对照组。两组患者均接受RBP、HBA1c、CysC、ACR测定并比较,重点分析联合检测RBP、HBA1c、CysC、ACR对T2DM早期肾损伤的诊断价值。结果观察组RBP、HBA1c、CysC、ACR水平均比对照组高,有统计学差异(P<0.05);绘制ROC曲线,结果显示,RBP、HBA1c、CysC、ACR水平诊断T2DM早期肾损伤的AUC均大于0.80,诊断价值理想,且以联合诊断价值更佳。结论RBP、HBA1c、CysC、ACR联合诊断T2DM早期肾损伤价值较高,若RBP、HBA1c、CysC、ACR过表达,则提示T2DM早期肾损伤风险高,应采取积极的治疗措施。 Objective To explore the diagnostic value of combined detection of retinol binding protein(RBP),glycosylated hemoglobin(HBAlc),cystatin C(CysC),and the ratio of urinary microalbumin to creatinine(ACR)for early renal injury in type 2 diabetes(T2DM).Methods Retrospective analysis was conducted to collect the clinical data of 40 patients with early renal injury of T2DM admitted to the hospital from January 2021 to January 2022 as the observation group,and 40 patients with T2DM admitted to the hospital at the same time as the control group.RBP,HBAlc,CysC and ACR were measured and compared in both groups.The diagnostic value of combined detection of RBP,HBAlc,CysC and ACR for early renal injury in T2DM was emphatically analyzed.Results The levels of RBP,HBA1c,CysC and ACR in the observation group were higher than those in the control group(P<0.05);the ROC curve was drawn,and the Results showed that the AUC of RBP,HBA1c,CysC,ACR levels in the diagnosis of early renal injury in T2DM was>0.80,with ideal diagnostic value,and the combined diagnostic value was better.Conclus ion The combination of RBP,HBAlc,CysC and ACR is of high value in the diagnosis of early renal injury in T2DM.If the overexpression of RBP,HBAIc,CysC and ACR indicates that the risk of early renal injury in T2DM is high,active treatment measures should he taken.
