微创血肿清除术治疗对脑深部功能区出血患者神经功能、日常生活能力及神经因子水平的影响

Effect of Minimally Invasive Hematoma Removal on Nerve Function,Daily Living Ability,and Nerve Factor Level in Patients with Deep Cerebral Hemorrhage

摘要 目的探讨脑深部功能区出血患者给予微创血肿清除术治疗对神经功能、日常生活能力及神经因子水平的影响。方法选取2020年5月至2021年6月虞城县人民医院收治的72例脑深部功能区出血患者作为研究对象,按照随机数字表法分成观察组(36例)与对照组(36例),对照组实施常规开颅手术治疗,观察组实施微创血肿清除术治疗,比较两组手术指标、神经功能、日常生活能力及神经因子水平。结果观察组手术时间、切口长度短于对照组,术中出血量少于对照组,血肿清除率高于对照组,差异有统计学意义(P<0.05)。两组术后美国国立卫生院神经功能缺损评分量表(NIHSS)评分均明显降低,且观察组低于对照组,差异有统计学意义(P<0.05)。两组术后Bathel指数(BI)评分均明显提高,且观察组高于对照组,差异有统计学意义(P<0.05)。两组术后S-100β蛋白(S-100β)、神经元特异性烯醇化酶(NSE)、胶质纤维酸性蛋白(GFAP)水平均明显降低,且观察组低于对照组,差异有统计学意义(P<0.05)。结论脑深部功能区出血患者给予微创血肿清除术治疗,能够改善手术指标,促进患者神经功能恢复、提高日常生活能力,调节神经因子水平。 Objective To investigate the effects of minimally invasive hematoma removal on nerve function,daily living ability,and nerve factor level in patients with deep cerebral functional area hemorrhage.Methods A total of 72 patients with deep cerebral hemorrhage admitted to Yucheng County People's Hospital from May 2020 to June 2021 were selected as research objects.According to the random number table method,they were divided into an observation group(36 cases)and a control group(36 cases).The control group received conventional craniotomy,and the observation group received minimally invasive hematoma removal.The operative indexes,nerve function,daily living ability,and nerve factor levels of the two groups were compared.Results The operation time and incision length of the observation group were shorter than that of the control group,the amount of intraoperative blood loss was less than that of the control group,and the clearance rate of hematoma was higher than that of the control group,the difference was statistically significant(P<0.05).The national institute of health neurological deficit scale score(NIHSS)of the two groups decreased significantly,and the observation group was lower than the control group,and the difference was statistically significant(P<0.05).Barthel index(BI)score was significantly improved in both groups,and the observation group was higher than the control group,the difference was statistically significant(P<0.05).The levels of S-100βprotein(S-100β),neuron-specific enolase(NSE),and glial fibrinous acid protein(GFAP)in 2 groups were significantly decreased after surgery,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).Conclusion Minimally invasive hematoma removal in patients with deep cerebral hemorrhage can improve the surgical indicators,promote the recovery of neurological function,improve the ability to do daily living,and regulate the level of neurological factors.
