红细胞参数和转铁蛋白含量在贫血鉴别诊断中的价值

The Value of red Blood Cell Parameters and Transferrin Content in the differential Diagnosis of Anemia

目的观察针对贫血患者使用血液检验红细胞参数和转铁蛋白含量的临床应用价值。方法临床研究对象选取南阳市中心医院2021年3月至2022年3月收治的80例贫血患者和80例健康志愿者,80例志愿者为对照组,80例贫血患者为观察组。两组均进行血液检查,对两组患者的红细胞参数及转铁蛋白含量进行观察,且将观察组患者不同贫血类型和不同程度贫血的红细胞参数及转铁蛋白含量进行对比。结果观察组血红蛋白(HB)、红细胞平均体积(MCV)、红细胞计数(RBC)及平均红细胞血红蛋白含量(MCH)参数均低于对照组,且红细胞分布宽度(RDW)和转铁蛋白(TRF)指数均高于对照组,差异有统计学意义(P<0.05)。缺铁性贫血HB、RBC、TRF指标高于地中海性贫血、巨幼细胞性贫血及再生障碍性贫血,而MCV、RDW及MCH指标均低于其他类型贫血,差异有统计学意义(P<0.05)。HB、MCV、RBC、MCH指标对比,结果为极重度<重度<中度<轻度;RDW与TRF指标对比,结果为极重度>重度>中度>轻度,差异有统计学意义(P<0.05)。结论针对贫血患者采用血液检验红细胞参数及转铁蛋白含量的鉴别诊断方式,操作简单且鉴别诊断结果精准,一方面能够及时有效对贫血患者进行诊断,另一方面能够准确判断患者贫血类型,为患者不同类型贫血的诊治提供准确可靠的诊治方案,具有临床使用价值。

Objective To observe the clinical application value of using blood tests for red blood cell parameters and transferrin content in anemia patients.Method Clinical research subjects were selected from 80 anemia patients and 80 healthy volunteers admitted to Nanyang Central Hospital from March 2021 to March 2022.80 volunteers served as the control group,and 80 anemia patients served as the observation group.Both groups underwent blood tests to observe the red blood cell parameters and transferrin content of the two groups of patients,and the red blood cell parameters and transferrin content of the observation group patients with different types and degrees of anemia were compared.Results The parameters of hemoglobin(HB),mean corpuscular volume(MCV),red blood cell count(RBC),and mean corpuscular hemoglobin content(MCH)in the observation group were lower than those in the control group,and the red blood cell distribution width(RDW)and transferrin(TRF)index were higher than those in the control group,with statistically significant differences(P<0.05).The HB,RBC,and TRF indicators of iron deficiency anemia were higher than those of thalassemia,megaloblastic anemia,and aplastic anemia,while the MCV,RDW,and MCH indicators were lower than other types of anemia,with statistical significance(P<0.05).The comparison of HB,MCV,RBC,and MCH indicators showed extremely severe<severe<moderate<mild;The comparison between RDW and TRF indicators showed that the results were extremely severe>severe>moderate>mild,with a statistically significant difference(P<0.05).Conclusion The differential diagnosis method of using blood test red blood cell parameters and transferrin content for anemia patients is simple in operation and accurate in differential diagnosis results.On the one hand,it can timely and effectively diagnose anemia patients,and on the other hand,it can accurately determine the type of anemia in patients,providing accurate and reliable diagnosis and treatment plans for different types of anemia in patients,which has clinical value.
