雷公藤多苷对老年糖尿病肾病患者的疗效观察

Effects of tripterygium wilfordii polyglycosides on elderly patients with diabetes nephropathy

目的 探讨雷公藤多苷(tripterygium glycosides, TWG)对老年糖尿病肾病(diabetic nephropathy, DN)患者的临床疗效以及血糖、血脂的影响。方法 选择122例老年DN患者,随机分为以TWG组(剂量为TWG剂量为1 mg/kg·d)和缬沙坦治疗对照组2组,每组61例,治疗8周,观察2组患者的临床疗效及血脂、血糖和血压等指标。结果 (1) 2组治疗后尿微量白蛋白排泄率较治疗前下降(P<0.001),TWG降低比缬沙坦明显,差异有统计学意义(P<0.001);(2)2组治疗后血糖和糖化血红蛋白较治疗前下降(P<0.001),但组间比较差异无统计学意义(P>0.05)。(3) 2组治疗前后血LDL-C差异均无统计学意义(P>0.05)。结论 TWG治疗老年DN患者有较好的临床效果,能明显降低尿微量白蛋白的排泄,有降低血糖作用,对血脂无不良影响。

Objective To explore the therapeutic effects of tripterygium wilfordii polyglycosides(TWG)on elderly pa⁃tients with diabetes nephropathy(DN)and its effect on blood glucose and blood lipid.Methods A total of 122 elderly patients with DN were randomly divided into TWG group[TWG dose was 1 mg/(kgod)]and control group(treated with valsartan),with 61 cases in each group.Both groups were treated for 8 weeks.The curative effect,blood lipid,blood glucose and blood pressure of the two groups were observed and compared.Results(1)After treatment,the urinary microalbumin excretion rate in both groups decreased compared to that before treatment(P<0.001),and the decrease in the TWG group was more signifi⁃cant than that in the control group,with a statistically significant difference(P<0.001).(2)After treatment,blood glucose and glycosylated hemoglobin in the two groups decreased compared to those before treatment(P<0.001),but there was no difference between the two groups(P>0.05).(3)There was no significant difference in blood LDL⁃C between the two groups before and after treatment(P>0.05).Conclusion TWG has a good clinical effect in the treatment of elderly patients with DN and it can significantly reduce the excretion of urinary microalbumin and blood sugar,and has no adverse effect on blood lipid.
