静配中心对儿科分剂量用药调配的质量控制探讨

Study on quality control of dosage dispensing in pediatrics by intravenous drug dispensing center

目的探讨静脉药物调配中心儿科分剂量药品精准调配的质控方法,为临床患儿提供优质服务,确保输液质量安全。方法收集本院儿科住院病房常用分剂量药品,按照药品的规格、稀释要求,溶解特点等,采用倍数换算法进行质控实践。结果分剂量溶液型注射液依据含量(g/mL,mg/mL)换算为相应体积;注射用无菌粉末加入溶媒后,药品剂量≤0.5g药液增量为0~0.2mL,剂量0.60~0.75g药液增量0.3~0.4mL,剂量1g药液增量为0.5mL,剂量1.2~1.25g药液增量为0.6~0.8mL,剂量1.5g药液增量为1mL,剂量4.5g药液增量为2.8mL;分剂量注射用无菌粉末溶解后体积>医嘱分剂量大输液体积×(±5%),医嘱分剂量大输液再次分剂量,分剂量注射用无菌粉末溶解后体积≤医嘱分剂量大输液体积×(±5%),医嘱分剂量大输液不需再次分剂量。结论对儿科分剂量药品调配过程进行质量控制,有利于儿科精准用药。

Objective To explore the quality control method of precise dispensing of pediatric sub-dose drugs in intravenous drug dispensing center,so as to provide high quality service for clinical children and ensure the quality and safety of infusion.Methods It collected sub-dose drugs in common use of pediatric patient ward in our hospital according to the specifications,dilution requirements and dissolution characteristics of the drugs,multiple conversion algorithm was used in quality control practice.Results According to the content(g/mL,mg/mL)of the solution-type injection,the volume of the solution-type injection was converted into the corresponding volume,and the dosage of the sterile powder for injection was≤0.5 g,the increment of liquid medicine was 0-2 mL.Dosage 60-0.75 G liquid medicine increment 0.3~0.4 mL,the increment of 1 g liquid medicine was 0.5 mL,the increment of 1.2~1.25 g liquid medicine was 0.6~0.8 mL,the increment of 1.5 g liquid medicine was 1 mL,the increment of 4.5 g liquid medicine was 2.8 mL;The volume after dissolution of sterile powder for sub-dose injection>the doctor's order volume of sub-dose infusion solution×(±5%);Doctor ordered split-dose large infusion did not need to split dosage again.Conclusions The quality control of the dispensing process of pediatric sub-dose drugs is conducive to drug accurate use of pediatrics.
