紫杉醇联合替吉奥和奥沙利铂治疗晚期胃癌的疗效

Clinical effect of paclitaxel combined with tegafur,gimeracil and oteracil potassium and oxaliplatin against advanced gastric cancer

目的观察紫杉醇联合替吉奥和奥沙利铂治疗晚期胃癌的疗效。方法选取2020年1月—2022年6月于甘肃省人民医院接受治疗的67例晚期胃癌患者作为研究对象,按照随机数字表法分为对照组33例和试验组34例。对照组采用替吉奥联合奥沙利铂治疗,试验组在对照组基础上联合紫杉醇。2组治疗时间均为6个月。比较2组临床疗效,症状消失时间,治疗前及治疗6个月后肿瘤标志物[血清糖类抗原19-9(CA19-9)、癌胚抗原(CEA)]及日常生活活动能力量表(ADL)、改良Barthel指数量表(MBI)评分,不良反应。结果试验组治疗总有效率为97.06%,高于对照组的78.79%(χ^(2)=5.317,P=0.021)。试验组手足综合征、肝肾功能异常、活动障碍消失时间短于对照组(P<0.01)。治疗6个月后,2组血清CA19-9、CEA水平低于治疗前,ADL、MBI评分高于治疗前,且试验组降低/升高幅度大于对照组(P<0.01)。2组不良反应总发生率比较,差异无统计学意义(P>0.05)。结论紫杉醇联合替吉奥和奥沙利铂治疗晚期胃癌的效果确切,可快速改善临床症状,提高生活质量,且安全性较高。

Objective To observe the effect of paclitaxel combined with tegafur,gimeracil and oteracil potassium and oxaliplatin against advanced gastric cancer.Methods A total of 67 cases of advanced gastric cancer were selected from January 2020 to June 2022 in Gansu Provincial Hospital,and they were divided into the control group with 33 cases and the experimental group with 34 cases by random number table method.Patients of the control group were treated with tegafur,gimeracil and oteracil potassium and oxaliplatin,and patients of the experimental group were treated with paclitaxel on the basis of the control group.Both groups were treated for 6 months.Clinical effect,symptom disappearance time,tumor markers(serum CA19-9,CEA)and scores of ADL,MBI before and after 6 months of treatment were compared between the two groups,and incidence of adverse reactions of the two groups were observed.Results Total effective rate of the experimental group was 97.06%,which was higher than 78.79%of the control group(χ^(2)=5.317,P=0.021).Disappearing time of hand-foot syndrome,liver and kidney dysfunction,movement disorder of the experimental group were shorter than those of the control group(P<0.01).After 6 months of treatment,the two groups of serum levels of CA19-9,CEA were lower than those before treatment,scores of ADL,MBI were higher than those before treatment,and decreased/increased range of indicators in the experimental group were greater than those of the control group(P<0.01).There was no significant difference of the incidence of adverse reactions between the two groups(P>0.05).Conclusion Paclitaxel combined with tegafur,gimeracil and oteracil potassium and oxaliplatin have an exact effect for advanced gastric cancer,it can quickly improve the clinical symptoms of patients,promote the quality of life,and with high safety.
