全面护理对重症急性呼吸窘迫综合征患者血气指标的调节作用

Regulatory effect of comprehensive nursing on blood gas indexes in patients with severe acute respiratory distress syndrome

目的探讨全面护理在重症急性呼吸窘迫综合征(ARDS)患者中的应用效果。方法选取我院2021年1月至9月收治的86例ARDS患者,根据护理干预方式不同将其分为对照组(n=43,常规护理)和观察组(n=43,全面护理)。比较两组的干预效果。结果护理后,观察组的动脉血氧分压(PaO_(2))、pH、氧合指数(OI)及静态肺顺应性(Cst)高于对照组,二氧化碳分压(PaCO_(2))低于对照组(P<0.05)。护理后,观察组的血管生成素-2及心肌肌钙蛋白T水平低于对照组,硫化氢水平高于对照组(P<0.05)。观察组的发热缓解时间、机械通气治疗时间、住院时间短于对照组,并发症发生率低于对照组,舒适度评分高于对照组(P<0.05)。结论将全面护理应用于重症ARDS患者中有助于改善血气、呼吸动力学以及血清学指标,缩短症状持续及治疗时间,改善预后。

Objective To explore the application effect of comprehensive nursing in patients with severe acute respiratory distress syndrome(ARDS).Methods Eighty-six patients with severe ARDS admitted in our hospital from January to September 2021 were selected and divided into control group(n=43,routine nursing)and observation group(n=43,comprehensive nursing)according to different nursing intervention methods.The intervention effects of the two groups were compared.Results After nursing,arterial partial pressure of oxygen(PaO_(2)),pH,oxygenation index(OI)and static lung compliance(Cst)in the observation group were higher than those in the control group,while partial pressure of carbon dioxide(PaCO_(2))was lower than that in the control group(P<0.05).After nursing,the levels of angiopoietin-2 and cardiac troponin T in the observation group were lower than those in the control group,hydrogen sulfide level was higher than that in the control group(P<0.05).The remission time of fever,treatment time of mechanical ventilation and hospital stay in the observation group were shorter than those in the control group,the incidence of complications was lower than that in the control group,and the comfort score was higher than that in the control group(P<0.05).Conclusion The comprehensive nursing applied in patients with severe ARDS is helpful to improve the blood gas,respiratory dynamics and serological indexes,shorten duration of symptoms and treatment time,and improve the prognosis.
