基于循证理论的精细化干预结合造口袋更换口诀卡对结直肠癌根治术后肠造口患者病耻感及炎症因子水平的影响

Effects of refined intervention based on evidence-based theory combined with pocket replacement tips card on stigma and inflammatory factors levels in patients with enterostomy after radical resection of colorectal cancer

目的探讨基于循证理论的精细化干预结合造口袋更换口诀卡在结直肠癌根治术后肠造口患者中的应用效果。方法选取2019年2月至2021年3月本院收治的62例结直肠癌根治术后肠造口患者作为研究对象,按照随机数字表法将其分为对照组与观察组,各31例。对照组采用常规护理,观察组采用基于循证理论的精细化干预联合造口袋更换口诀卡。比较两组的干预结果。结果干预后,观察组的内在羞耻感、社会隔离、社会排斥、经济歧视评分低于对照组,差异具有统计学意义(P<0.05)。干预后,观察组的肠造口袋更换知识、造口护理理论知识、常见造口并发症护理评分高于对照组,差异具有统计学意义(P<0.05)。干预后,观察组的C-反应蛋白(CRP)及白细胞介素-1β(IL-1β)水平低于对照组,差异具有统计学意义(P<0.05)。结论基于循证理论的精细化干预结合造口袋更换口诀卡可减轻结直肠癌根治术后肠造口患者的病耻感,降低炎性因子水平,值得推广。

Objective To explore the application effect of refined intervention based on evidence-based theory combined with pocket replacement tips card in patients with enterostomy after radical resection of colorectal cancer.Methods A total of 62 patients with enterostomy after radical resection of colorectal cancer admitted in our hospital from February 2019 to March 2021 were selected as the research objects and divided into control group and observation group according to the random number table method,with 31 cases in each group.The control group was given routine nursing,while the observation group was given refined intervention based on evidence-based theory combined with pocket replacement tips card.The intervention effects of the two groups were compared.Results After intervention,the scores of internal shame,social isolation,social exclusion and economic discrimination in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After intervention,the scores of intestinal stoma bag replacement knowledge,theoretical knowledge of stoma nursing and nursing of common stoma complications in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).After intervention,the levels of C-reactive protein(CRP)and interleukin-1β(IL-1β)in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Refined intervention based on evidence-based theory combined with pocket replacement tips card can reduce the stigma of patients with enterostomy after radical resection of colorectal cancer,and reduce the levels of inflammatory factors,which is worthy of promotion.
