基于三维数字化模型评估上下颌牙弓宽度与性别的相关性

Assessing the correlation between maxillary and mandibular dental arch width and gender based on 3D digital model

摘要 目的利用三维数字化测量方法对上下颌牙弓宽度进行测量,来评估上下颌牙弓宽度与性别的相关性。方法本研究收集120名成年人的上下颌三维数字模型,男女各60名,利用逆向工程软件,分别测量上下颌两尖牙间、两第一前磨牙间、两第二前磨牙间、两第一磨牙间和两第二磨牙间的宽度,并对其进行统计分析,建立判别函数,以评估上下颌各牙弓的宽度与性别的关系。通过ROC曲线分析,得出所有牙弓尺寸估计性别的可能性。用Garn等给出的方法计算性别二型性百分比。结果除了两下颌尖牙间宽度,其余上下颌各牙弓宽度的平均值男性均大于女性,且具有统计学差异(p<0.05),用其建立判别函数,鉴定准确率为59.2%~80%。其中,上颌两第二磨牙间的宽度对性别的判断效果最好,为80%。性别鉴定的准确率随着从尖牙到第二磨牙的远移而增加。ROC曲线分析表明上颌两第二磨牙间宽度最有可能估计性别,曲线下面积为0.887。上颌第二磨牙间宽度的性别二型性最高,达到91.8%。结论上下颌各牙弓间的宽度在不同的性别之间存在差异,尤其是上颌第二磨牙间的宽度,因此可考虑将其用于遗骸的性别鉴定。 Objective A three-dimensional(3D)digital measurement method was used to measure the width of the maxillary and mandibular dental arch and evaluate the correlation between the width of the maxillary and mandibular dental arch and gender.Methods In this study,3D digital models of the maxilla and mandible were collected from 120 adults,including 60 males and 60 females.Reverse engineering software was used to measure the inter canine distance of maxilla and mandible,the inter first premolar distance of maxilla and mandible,the inter second premolar distance of maxilla and mandible,the inter first molar distance of maxilla and mandible,the inter second molar distance of maxilla and mandible.Then,the statistical analysis was performed to achieve a discriminant function and to evaluate the relationship between the width of each dental arch and gender.Receiver Operating Characteristic(ROC)analysis was conducted to evaluate the potential of sexual differentiation by all the dental arch width.The percentage of sex dimorphism was calculated using the method given by Garn et al.Results Except for the width of between mandibular canines,the average values of the maxillary and mandibular arch widths in males were larger than those in females,and the difference was statistically significant(p<0.05).By ssing it to establish a discriminant function,the identification accuracy was 59.2%~80%.Among them,the distance between the maxillary second molars had the best effect to reflect gender differences.The accuracy of sex determination increased with distance from the canine to the second molar.The ROC curve analysis showed that the distance between the maxillary second molars was the best choice for gender judgement,and the area under the ROC curve was 0.887.The width between the maxillary second molars had the highest gender dimorphism at 91.8%.Conclusion The width between the maxillary and mandibular dental arches varies with different genders,especially the width between the maxillary second molars.Therefore,it can be considered for gender identification of human remains.
