原发性高血压患者血清sSema4D、IGF-1水平与左心室肥大的相关性分析

Correlation Analysis Between Serum sSema4D,IGF-1 Levels and Left Ventricular Hypertrophy in Patients with Essential Hypertension

目的 探讨原发性高血压患者血清可溶性重组蛋白信号4D(sSema4D)、胰岛素样生长因子-1(IGF-1)水平与左心室肥大(LVH)的相关性。方法 选取2020年6月至2021年7月我院收治的原发性高血压患者135例,根据超声心动图结果分为LVH组70例和非LVH组65例,另外选取同期年龄与性别相匹配的对照人群70例作为对照组。记录受试者基本临床资料,采用酶联免疫吸附试验法检测血清sSema4D、IGF-1水平。结果 与对照组比较,非LVH组和LVH组血清sSema4D、IGF-1水平升高(均P<0.05);且LVH组血清sSema4D、IGF-1水平高于非LVH组(P<0.05)。经多元Logistic回归模型分析,体质指数(P=0.017)、收缩压(P=0.003)、血清sSema4D水平(P<0.001)、血清IGF-1水平(P<0.001)升高是LVH的独立危险因素。利用Pearson相关系数分析和多元线性回归分析,对于原发性高血压患者,血清sSema4D、IGF-1水平与LVM、LVMI均呈正相关(P<0.05)。此外在LVH组患者中,血清sSema4D水平与血清IGF-1水平亦呈正相关(P=0.001)。结论 sSema4D和IGF-1与LVH发病密切相关,有望成为治疗或预防原发性高血压患者LVH发生的新靶点。

Objective To investigate the correlation between serum soluble semaphorin 4D(sSema4D),insulin-like growth factor-1(IGF-1)and left ventricular hypertrophy(LVH)in patients with essential hypertension.Methods From June 2020to July 2021,a total of 135 patients with essential hypertension were enrolled and divided into LVH group( n =70)and non-LVH group( n =65)according to Doppler echocardiography results.In addition,a 70 cases of age and sex matched control group were selected.Basic clinical data was recorded and serum sSema4D and IGF-1 levels were measured by enzyme-linked immunosorbent assay method. Results Comparing with the control group,the serum sSema4D and IGF-1 levels in LVH group and non LVH group were higher(all P <0.05),and the serum sSema4D and IGF-1 levels in LVH group were higher than those in non LVH group( P <0.05).Multivariate Logistic regression model showed that body mass index( P =0.017),systolic blood pressure( P =0.003),serum sSema4D level( P <0.001)and serum IGF-1 level( P <0.001)were the independent risk factors of LVH.Using Pearson correlation coefficient analysis and multivariate linear regression analysis,the serum sSema4D and IGF-1 levels were positively correlated with LVM and LVMI in patients with essential hypertension( P <0.05).In addition,serum sSema4D level was positively correlated with serum IGF-1 level in LVH Group( P =0.001). Conclusion sSema4D and IGF-1 are closely related to the pathogenesis of LVH,which is expected to be a new target for the treatment or prevention of LVH in patients with essential hypertension.
