单通道经皮肾镜联合输尿管软镜碎石术对鹿角形肾结石患者结石清除率的影响

Effect of Single-Channel Percutaneous Nephroscopy Combined with Flexible Ureteroscopic Lithotripsy on Stone Clearance Rate in Patients with Staghorn-Shaped Kidney Stones

目的:分析单通道经皮肾镜联合输尿管软镜碎石手术方案,对鹿角形肾结石患者结石清除效果的影响。方法:随机选取102例鹿角形肾结石患者,根据患者手术治疗方案的不同,将其分为单通道组与多通道组。观察并对比两组患者的临床指标、结石清除率与术后并发症发生情况。结果:单通道组患者的手术用时、肾造瘘管留置时间与住院用时均短于多通道组(P<0.05);单通道组患者术后24h及术后1个月的结石清除率(80.77%,90.38%),与多通道组患者术后24h及术后1个月的结石清除率(82.00%,94.00%)相比,均无明显差异(P>0.05);单通道组患者Clavien-Dindo 2级术后并发症的发生率较多通道组患者更低(P<0.05)。结论:在鹿角形结石患者的结石清除效果上,单通道经皮肾镜联合输尿管软镜碎石术具有与多通道经皮肾镜碎石术相近的清除效果,同时,有利于缩短患者的手术治疗时间与住院时间,可进一步降低患者的术后并发症发生风险。

Objective:To analyze the effect of single-channel percutaneous nephrolithotomy combined with flexible ureteroscopic lithotripsy on stone removal in patients with staghorn-shaped kidney stones.Methods:A total of 102 patients with staghorn-shaped kidney stones were randomly selected and divided into single-channel group and multi-channel group according to different surgical treatment plans.The clinical indicators,stone clearance rate and postoperative complications of the two groups were observed and compared.Results:The operation time,nephrostomy tube indwelling time and hospitalization time of the patients in the single-channel group were all shorter than those in the multi-channel group(P<0.05).There was no significant difference in the stone removal rate(80.77%,90.38%)of patients in the single-channel group at 24 hours and 1 month after operation compared with those in the multi-channel group at 24 hours and 1 month after operation(82.00%,94.00%)(P>0.05).The patients in the single-channel group had Clavien-Dindo grade 2 postoperative The incidence of complications was lower in patients with more channels(P<0.05).Conclusion:In terms of stone removal effect in patients with staghorn calculi,single-channel percutaneous nephrolithotomy combined with flexible ureteroscopic lithotripsy has a similar clearing effect to multi-channel percutaneous nephrolithotomy,and it can ensure the stone removal effect of patients.At the same time,it is beneficial to shorten the surgical treatment time and hospitalization time of patients,which can further reduce the risk of postoperative complications of patients.
