比较研究微型钢板、克氏针应用于掌指骨开放性骨折急诊治疗中价值

To Compare the Application Value of Micro Plate and Kirschner Wire in Emergency Treatment of Open Metacarpal and Phalangeal Fractures

目的:在掌指骨开放性骨折患者的急诊治疗中分别应用微型钢板、克氏针治疗,比较其应用价值。方法:选取2020年2月~2022年2月在本院治疗的40例掌指骨骨折患者。采取随机数字表法,将其分为两组。两组均接受内固定手术治疗,对照组20例应用克氏针治疗,观察组20例应用微型钢板。比较两组围术期及恢复指标、治疗效果、掌指骨功能评分及并发症情况。结果:与对照组比较,观察组患者的手术、住院时间较长,但骨折愈合时间较短,差异均有统计学意义(P<0.05);观察组患者优良率与对照组比较,差异有统计学意义(95.00%,70.00%,P<0.05);术后6个月,观察组患者的掌指骨功能评分明显高于对照组,差异有统计学意义(P<0.05);观察组并发症发生率与对照组比较,差异有统计学意义(5.00%,30.00%,P<0.05)。结论:克氏针治疗掌指骨开放性骨折的手术及住院时间更短,但微型钢板治疗可获得更好的恢复效果,且能够缩短骨折愈合时间,减少并发症发生。

Objective:To compare the application value of micro plate and Kirschner wire in the emergency treatment of patients with open metacarpal and phalangeal fractures.Methods:From February 2020 to February 2022,40 patients with metacarpal and phalangeal fractures treated in our hospital were selected.They were divided into two groups by random number table method.The control group(20 cases)was treated with Kirschner wire,and the observation group(20 cases)was treated with micro plate.The perioperative period and recovery index,treatment effect,metacarpal and phalangeal function score and complications were compared between the two groups.Results:Compared with the control group,the operation and hospitalization time of the observation group were longer,but the fracture healing time was shorter,and the differences were statistically significant(P<0.05).The excellent and good rate of the observation group was 95.00%,compared with 70.00%of the control group,and the difference was statistically significant(P<0.05).At 6 months after operation,the metacarpal and phalangeal function score of the observation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).The incidence of complications in the observation group was 5.00%,compared with 30.00%in the control group,and the difference was statistically significant(P<0.05).Conclusion:In the treatment of open metacarpal and phalangeal fractures,Kirschner wire treatment has shorter operation and hospital stay,while micro plate treatment can obtain better recovery effect,shorten fracture healing time and reduce the incidence of complications.
