2型糖尿病合并隐匿性冠心病的心电图分析

ECG Analysis of Type 2 Diabetes Mellitus Complicated with Occult CHD

目的:比较2型糖尿病合并隐匿性冠心病诊断中平板运动试验与动态心电图检查的效果。方法:从福建医科大学附属泉州第一医院心内科收治的2型糖尿病患者中随机抽取87例作为研究对象,入院时间均介于2021年3月~2022年3月,均接受冠状动脉造影检查,检查结果提示,有43例2型糖尿病患者未合并隐匿性冠心病(对照组),44例2型糖尿病患者并合并隐匿性冠心病(实验组),随后对两组患者展开动态心电图检查,分析两组患者动态心电图检查特点。结果:实验组患者动态心电图异常率明显高于对照组(P<0.05);两组患者窦性心动过速/缓、房室肥大、期前收缩及肢体导联低电压发生率之间不存在明显差异(P>0.05);相较于对照组ST-T改变、传导异常、病理Q波发生率,实验组明显更高(P<0.05)。结论:2型糖尿病合并隐匿性冠心病患者心电图异常率相对更高,主要体现在传导异常、ST-T改变及病理Q波增多。

Objective:To compare the effect of tablet exercise test and Holter electrocardiogram examination in the diagnosis of type 2 diabetes mellitus with occult coronary heart disease.Methods:87 patients with type 2 diabetes who were admitted to the Department of Cardiology of Quanzhou First Hospital affiliated to Fujian Medical University were randomly selected as research objects.The admission time was from March 2021 to March 2022.All patients received coronary angiography.The examination results showed that 43 patients with type 2 diabetes were not complicated with occult coronary heart disease(control group),and 44 patients with type 2 diabetes were complicated with occult coronary heart disease(experimental group).Then the dynamic ECG examination was carried out for the two groups of patients,and the characteristics of dynamic ECG examination were analyzed.Results:The abnormal rate of Holter electrocardiogram in the experimental group was significantly higher than that in the control group(P<0.05);There was no significant difference between sinus tachycardia/bradycardia,AV hypertrophy,prephase contraction,and low voltage incidence of limb leads in the two groups(P>0.05);Compared with the control group,compared with ST-T changes,conduction abnormalities,and pathological Q-wave incidence,the experimental group was significantly higher(P<0.05).Conclusion:The abnormal rate of electrocardiogram in patients with type 2 diabetes and insidious coronary heart disease is relatively higher,which is mainly reflected in conduction abnormalities,ST-T changes and increased pathological Q wave.
