小蓟饮子加减汤剂联合吡柔比星治疗非肌层浸润性膀胱癌术后患者的疗效评价

Efficacy Evaluation of Xiaoji Yinzi Plus or Minus Decoction Combined with Pirarubicin in Treatment of Postoperative Patients with Non-muscle Invasive Bladder Cancer

目的观察小蓟饮子联合吡柔比星治疗非肌层浸润性膀胱癌(non-muscle-invasive bladder cancer,NMIBC)术后的疗效评价。方法随机选取2018年1月-2021年12月于南京市江宁中医院泌尿科治疗的NMIBC患者80例,随机分为观察组和对照组,各40例。对照组给予注射用盐酸吡柔比星治疗,观察组联合小蓟饮子加减汤剂治疗,定期观察2年内患者肿瘤复发情况、尿常规主要指标、热结下焦症阳性率、外周血异常糖链糖蛋白(TAP)阳性率发生情况。结果两组患者治疗2年内肿瘤复发率比较,差异无统计学意义(P>0.05);观察组患者在治疗3、6、12个月红细胞低于对照组,在治疗6、12个月白细胞低于对照组,差异有统计学意义(P<0.05);治疗3、6、12个月,观察组热结下焦证阳性率分别为22.50%、10.00%、2.50%,TAP阳性率分别为52.50%、37.50%、20.00%,均明显低于对照组,差异有统计学意义(χ^(2)=5.495、5.000、4.507、4.381、4.053、4.713,P<0.05)。结论小蓟饮子汤剂口服可减轻非肌层浸润性膀胱癌患者术后吡柔比星灌注化疗引起的热结下焦型尿路刺激症状,改善肿瘤预后。

Objective To observe the effect of Xiaoji Yinzi combined with pirarubicin in the treatment of non-muscleinvasive bladder cancer(NMIBC).Methods Eighty patients with NMIBC treated in the Department of Urology,Nanjing Jiangning Hospital of Traditional Chinese Medicine from January 2018 to December 2021 were randomly selected and randomly divided into observation and control groups,40 patients each.And the control group was treated with pirarubicin hydrochloride for injection,and the observation group was added with Xiaoji Yinzi plus or minus decoction.The recurrence of tumor,the main indicators of urine routine,the positive rate of fever with lower energizer syndrome,and the positive rate of peripheral blood abnormal sugar chain glycoprotein(TAP)were observed regularly within 2 years.Results The rates of tumor recurrence within 2 years of treatment were compared between the two groups,the difference was not statistically significant(P>0.05).The erythrocyte and leukocyte in the observation group was lower than that in the control group at 3,6 and 12 months after treatment,the difference was statistically significant(P<0.05).After 3,6 and 12 months of treatment,the positive rate of fever with lower energizer syndrome in the observation group was 22.50%,10.00%and 2.50%respectively,and the positive rate of TAP was 52.50%,37.50%and 20.00%respectively,all were significantly lower than those in the control,the difference was statistically significant(χ^(2)=5.495,5.000,4.507,4.381,4.053,4.713,P<0.05).Conclusion The oral administration of Xiaoji Yinzi decoction can reduce the symptoms of pyretic and lower energizer type urinary tract irritation caused by postoperative chemotherapy with pirarubicin in patients with non-muscle invasive bladder cancer,and improve tumor prognosis.
