经掌侧入路锁定加压钢板固定术在老年桡骨远端骨折中的应用分析

Application of Volar Approach Locking Compression Plate Fixation in Elderly Patients with Distal Radius Fractures

目的 探讨经掌侧入路锁定加压钢板固定术在老年桡骨远端骨折(distal radius fracture,DRF)中的应用效果。方法 方便选取2019年7月—2021年6月菏泽市第六人民医院收治的96例老年DRF患者为研究对象,按照随机数表法将其分为两组,每组48例。研究组行经掌侧入路锁定加压钢板固定术,对照组行经背侧入路锁定加压钢板固定术。针对两组手术时间、手术出血量、骨折愈合时间、临床疗效以及术后并发症情况进行比较。结果 研究组骨折愈合时间(9.02±2.23)周较对照组(12.03±1.98)周短,差异有统计学意义(t=6.993,P<0.05);手术时间(65.45±3.48)min较对照组(60.65±4.52)min长,差异有统计学意义(t=5.830,P<0.05);手术出血量(66.45±0.52)mL与对照组(66.50±0.80)mL对比,差异无统计学意义(P>0.05)。在治疗的优良率对比中,研究组(89.58%)与对照组(87.50%)比较,差异无统计学意义(P>0.05)。在术后并发症发生率对比中,研究组(2.08%)较对照组(16.67%)低,差异有统计学意义(χ^(2)=4.414,P<0.05)。结论 老年DRF患者行掌侧入路锁定加压钢板固定术治疗效果可靠,与经背侧入路相比,能够有效缩短康复速度,降低并发症风险。

Objective To investigate the effect of volar approach locking compression plate fixation in elderly patients with distal radius fracture(DRF).Methods A total of 96 elderly patients with DRF admitted to the Sixth People's Hospital of Heze City from July 2019 to June 2021 were conveniently selected as research objects and divided into two groups according to random number table method,with 48 cases in each group.The study group underwent volar approach locking compression plate fixation and the control group underwent dorsal approach locking compression plate fixation.Operation time,operation bleeding,fracture healing time,clinical efficacy and postoperative complications were compared between the two groups.Results The fracture healing time of the study group(9.02±2.23)weeks was shorter than that of the control group(12.03±1.98)weeks,and the difference was statistically significant(t=6.993,P<0.05);the operation time(65.45±3.48)min was longer than that of the control group(60.65±4.52)min,and the difference was statistically significant(t=5.830,P<0.05);there was no statistically significant difference in the operation blood loss(66.50±0.80)mL between the control group(66.45±0.52)mL(P>0.05).In the comparison of excellent and good rate of treatment,89.58%in the study group and 87.50%in the control group showed no statistically significant difference(P>0.05).In the comparison of the incidence of postoperative complications,2.08%in the study group was significantly lower than 16.67%in the control group,and the difference was statistically significant(χ^(2)=4.414,P<0.05).Conclusion The volar approach locking compression plate fixation in elderly patients with DRF has a reliable treatment effect.Compared with the dorsal approach,it can effectively shorten the recovery speed and reduce the risk of complications.
