鼻内镜鼻窦手术联合鼻甲射频消融术治疗慢性鼻窦炎的临床效果

Clinical Effect of Nasal Endoscopic Sinus Surgery Combined with Radiofrequency Ablation of Nasal Turbinate in the Treatment of Chronic Rhinosinusitis

目的 观察鼻内镜鼻窦手术(endoscopic sinus surgery,ESS)联合鼻甲射频消融术对慢性鼻窦炎(chronic rhinosinusitis,CRS)患者的临床效果。方法 简单随机选取2019年1月—2021年1月漳州市第三医院接诊的100例CRS患者,随机分为观察组(50例,行ESS+鼻甲射频消融术)、对照组(50例,行ESS),比较两组鼻腔黏液纤毛清除功能、临床症状评分、嗅觉功能、临床有效率、并发症发生情况。结果 术前,两组鼻腔黏液纤毛清除功能、临床症状评分、嗅觉功能比较,差异无统计学意义(P>0.05);术后3个月,观察组鼻腔黏液纤毛清除功能、临床症状评分、嗅觉功能较对照组更佳,差异有统计学意义(P<0.05)。观察组临床有效率(96.00%)高于对照组(80.00%),差异有统计学意义(χ^(2)=6.061,P=0.014);观察组并发症发生率(8.00%)与对照组(6.00%)比较,差异无统计学意义(χ^(2)=0.000,P=1.000)。结论 对CRS患者行ESS+鼻甲射频消融术,能够改善鼻腔黏液纤毛清除功能、嗅觉功能,减轻临床症状,疗效确切,且不会提升并发症风险。

Objective To observe the clinical effect of nasal endoscopic sinus surgery(ESS)combined with radiofrequency ablation of nasal turbinate in patients with chronic rhinosinusitis(CRS).Methods 100 patients with CRS admitted to Zhangzhou Third Hospital from January 2019 to January 2021 were simple randomly selected and randomly divided into an observation group(50 cases,and performed ESS+radiofrequency ablation of nasal turbinate)and a control group(50 cases,and performed ESS).The nasal mucus cilia clearance function,clinical symptom score,olfactory function,clinical efficiency,occurrence of complications were compared between the two groups.Results Preoperatively,there were no statistically significant differences in the nasal mucus cilia clearance function,clinical symptom score and olfactory function between the groups(P>0.05).At 3 months after surgery,the nasal mucus cilia clearance function,clinical symptom score and olfactory function were better in the observation group,and the difference was statistically significant(P<0.05).The observation group of clinical efficiency(96.00%)was higher than the control group(80.00%),and the difference was statistically significant(χ^(2)=6.061,P=0.014).The observation group of incidence of complications(8.00%)was compared with that in the control group(6.00%),the difference was not statistically significant(χ^(2)=0.000,P=1.000).Conclusion Performing ESS+radiofrequency ablation of nasal turbinate in patients with CRS can improve nasal mucus cilia clearance function,olfactory function,and reduce clinical symptoms with definite efficacy and without elevating the risk of complications.
