摘要
目的分析新型冠状病毒肺炎疫情下某大型三级脑专科医院临床药师接受医护人员用药咨询情况相关数据,为疫情常态化管理下临床药师接受医护人员用药咨询提供参考及借鉴,提高临床药学服务水平。方法对2020年1月至2021年12月广东三九脑科医院临床药师接受医生护士咨询并完整记录的用药咨询,按咨询者、咨询方式、咨询问题内容和咨询药物类别进行分类,利用帕累托图对咨询问题内容进行分析。结果新型冠状病毒肺炎疫情下临床药师接受医护人员药物咨询共634例,以医生咨询占比高,共590例(93.05%),其中住院医师咨询例数最多(302例,51.13%),其次分别为主治医师(250例,42.38%)、主任/副主任医师(38例,6.49%),护士44例(6.94%)。外科423例(66.67%),内科211例(33.28%)。咨询方式主要为电话、面对面、微信,分别为325例(51.26%)、172例(27.13%)和137例(21.61%)。咨询问题内容以药物治疗方案168例(26.50%)、药物选择116例(18.30%)、用法用量102例(16.09%)、适应证75例(11.83%)为主。咨询药物类别以抗感染用药405例(63.88%)、中枢神经系统用药56例(8.83%)、消化系统用药42例(6.62%)排名前3位。结论新型冠状病毒肺炎疫情常态化管理下,医护用药咨询以医生咨询抗感染治疗方案、药物选择及药物用法用量占比最高,外科医师较内科医师咨询比例更高,通过接受用药咨询,临床药师可进一步积累经验,提高临床用药知识水平,体现临床药师专业价值。
Objective To provide reference for clinical pharmacists to receive medication consultation from medical staff under the normalized management of the epidemic,and to improve clinical pharmacy service level by analyzing the data of clinical pharmacists receiving medication consultation from medical staff in Guangdong 999 Brain Hospital during COVID-19 pandemic.Methods From January 2020 to December 2021,drug consultations,which were received and recorded completely by the clinical pharmacists from doctors and nurses of a large tertiary brain special hospital,were classified according to the consultants,consultation methods,consultation questions and consultation drug categories.Pareto chart was used to analyze the content of consulting questions.Results A total of 634 cases of drug consultations from medical staff were received by clinical pharmacists during COVID-19.Drug consultations from doctors,with a total of 590 cases(93.05%),accounted for the highest proportion of consultations,of which the proportion from resident doctors was the largest(302 cases,51.13%),followed by attending doctors(250 cases,42.38%),chief/deputy chief doctors(38 cases,6.49%),and 44 were from nurses(6.94%).There were 423 cases(66.67%)from surgical department and 211 cases(33.28%)from internal medicine department.The medication consultation was mainly by telephone,face-to-face and WeChat,which accounted for 51.26%(n=325),27.13%(n=172)and 21.61%(n=137),respectively.The content of medication consultation mainly included 168 cases of drug treatment plan(26.50%),116 cases of drug selection(18.30%),102 cases of usage and dosage(16.09%),and 75 cases of indications(11.83%).In terms of drug category,405 cases(63.88%)of anti-infection drugs,56 cases of central nervous system drugs(8.83%),and 42 cases of digestive system drugs(6.62%)ranked the top three of the drug category consultation received by clinical pharmacists.Conclusion Under the normalized management of COVID-19 pandemic,medication consultations from doctors and nurses are mainly about anti-infection treatment,drug selection and drug usage and dosage from doctors,which accounts for the highest proportion.Medication consultation from surgeons is more than that from physicians.By accepting drug consultations,clinical pharmacists can further accumulate experience and improve the level of clinical medication knowledge,showing their professional value.
