加味消痈解毒汤联合西医治疗急性肾脓肿的疗效与安全性

Efficacy and Safety of Modified Xiaojia Jiedu Decoction Combined with Western Medicine in the Treatment of Acute Renal Abscess

目的观察加味消痈解毒汤联合西医治疗急性肾脓肿的疗效与安全性。方法将78例急性肾脓肿患者采用随机数字表法分为观察组与对照组各39例。对照组患者予西医常规治疗,观察组在对照组基础上予加味消痈解毒汤治疗。两组均治疗2周后比较临床疗效、主要症状评分、实验室指标及临床症状和阳性体征消失时间。结果观察组患者的临床总有效率为94.87%,明显高于对照组的79.49%(P<0.05)。治疗后,两组患者的肾区叩痛、腰肌紧张、寒战不适、发热症状评分较治疗前均下降(P<0.05),观察组患者治疗后主要症状评分显著低于对照组(P<0.05)。两组患者的白细胞计数(WBC)、C反应蛋白(CRP)、降钙素原(PCT)水平较治疗前均下降(P<0.05),观察组水平显著低于对照组(P<0.05)。观察组患者高热、肾区叩痛、膀胱刺激症状消失时间明显短于对照组(P<0.05)。结论加味消痈解毒汤联合西医治疗急性肾脓肿的疗效与安全性均良好,可进一步改善患者的临床症状,缩短临床症状和阳性体征消失时间,降低WBC、CRP、PCT水平。

Objective:To investigate the efficacy and safety of modified Xiaojia Jiedu Decoction combined with western medicine in the treatment of acute renal abscess.Methods:A total of 78 patients with acute renal abscess were divided into an observation group and a control group by random number table method,with 39 cases in each group.The patients in the control group were treated with conventional western medicine,and the patients in the observation group were treated with modified Xiaojia Jiedu Decoction on the basis of the treatment in the control group.After 2 weeks of treatment,the clinical efficacy,main symptom scores,laboratory indicators,and disappearance time of clinical symptoms and signs were compared between the two groups.Results:After treatment,the total clinical effective rate of the observation group was 94.87%,which was significantly higher than 79.49%of the control group,with a statistically significant difference(PV 0.05).After treatment,the scores of percussion pain in the kidney area,psoas muscle tension,chills and discomfort,and fever symptoms in the two groups were all decreased compared with those before treatment(P<0.05),and the main symptom scores in the observation group after treatment were significantly lower than those in the control group(P<0.05).The levels of WBC,CRP and PCT in the two groups were decreased compared with those before treatment(P<0.05),and the levels of WBC,CRP and PCT in the observation group were significantly lower than those in the control group after treatment group(P<0.05).The disappearance time of high fever,renal percussion pain and bladder irritation symptoms in the observation group was significantly shorter than that in the control group,and the differences were statistically significant(P<0.05).Conclusion:Modified Xiaojia Jiedu Decoction combined with western medicine has good efficacy and safety in the treatment of acute renal abscess,which can further improve the clinical symptoms of patients,shorten the disappearance time of clinical symptoms and signs,and reduce the levels of WBC,CRP and PCT.
