鼻舒宣痹汤联合针灸治疗急性鼻窦炎(肺经风热证)临床观察

Effects of Bishu Xuanbi Decoction combined with Acupuncture in Patients with Acute Rhinosinusitis of Lung Meridian Wind-heat Syndrome

目的观察鼻舒宣痹汤联合针灸治疗对急性鼻窦炎(ARS)(肺经风热证)患者鼻纤毛传输功能的影响。方法将于本院就诊的82例患者按随机数字表法分为观察组与对照组各41例。对照组在西医常规治疗的基础上增加针灸治疗,观察组患者在对照组治疗基础上增加鼻舒宣痹汤。两组患者均以1周为1个疗程,治疗2个疗程后比较中医证候疗效、主要症状评分、鼻纤毛传输功能以及炎症指标。结果治疗2个疗程后,观察组中医证候总有效率为90.24%,显著高于对照组的70.73%(P<0.05)。治疗后,两组鼻涕、鼻塞、嗅觉减退症状评分较治疗前明显下降(P<0.05),观察组下降程度更显著(P<0.05);两组MCC、MTR较治疗前明显升高(P<0.05),观察组升高程度更显著(P<0.05);两组白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平较治疗前明显下降(P<0.05),观察组下降程度更显著(P<0.05)。结论鼻舒宣痹汤联合针灸治疗ARS(肺经风热证)的疗效良好,可进一步改善患者的临床症状,提高鼻纤毛传输功能,降低炎症指标水平。

Objective:To investigate the effect of Bishu Xuanbi Decoction combined with acupuncture on nasal ciliary transmission function in patients with acute rhinosinusitis(ARS)of lung meridian wind-heat syndrome.Methods:A total of 82 patients with ARS o£lung meridian wind-heat syndrome were divided into an observation group(n=41)and a control group(n=41)by random number table method.The control group was treated with acupuncture and moxibustion on the basis of conventional western medicine,and the observation group was treated with Bishu Xuanbi Decoction on the basis of the control group.The two groups of patients took 1 week as a course of treatment.After 2 courses of treatment,the efficacy of TCM syndromes,main symptom scores,nasal ciliary transmission function and inflammatory indexes were compared.Results:After 2 courses of treatment,the total effective rate of TCM syndromes in the observation group was 90.24%,which was significantly higher than 70.73%in the control group(P<0.05).After treatment,the symptom scores of nasal discharge,nasal congestion and hypos-mia in the two groups were significantly decreased compared with those before treatment(P<0.05),and the degree of decline in the observation group was more significant(P<0.05).After treatment,MCC and MTR in the two groups were significantly increased compared with those before treatment(P<0.05),and the degree of increase in the observation group was more significant(P<0.05).After treatment,the levels of IL-6 and TNF-a in the two groups were significantly decreased compared with those before treatment(P<0.05),and the degree of decrease in the observation group was more significant(P<0.05).Conclusion:Bishu Xuanbi Decoction combined with acupuncture has good curative effect in the treatment of ARS patients with lung meridian wind-heat syndrome,which can further improve the clinical symptoms of patients,improve the nasal ciliary transmission function,and reduce the level of inflammatory indicators.
