北京市普仁医院多学科协作拔牙门诊患者的诊疗特点

Clinical characteristics of patients in multi-disciplinary team of tooth extraction in Beijing Puren Hospital

目的分析北京市普仁医院多学科协作(MDT)拔牙门诊患者诊疗特点与疗效,为医疗资源配置提供参考。方法回顾性分析2019年1月至2020年1月收治的230例MDT拔牙门诊患者的临床资料,分析患者的一般情况、就诊特点以及拔牙完成情况。结果调查人群平均年龄(70.1±12.4)岁,60岁以上占81.7%。牙周病、牙源性感染、无法修复的残冠残根是拔牙主要原因。全身合并症主要为高血压病、冠心病、糖尿病、心律失常,占83.6%;同时罹患两种以上合并症占比40%,复杂多样危险性大。MDT拔牙总完成率90.9%,55.9%的门诊患者3 d内、95.2%的门诊患者、81%的住院患者1周内完成拔牙,比较安全便利。结论普仁医院MDT拔牙有较好的临床疗效,应充分发挥综合医院资源优势,进一步合理配置以MDT拔牙为主的牙槽外科医疗资源,积极发展专科特色,满足老龄化社会医疗需求。

Objective To analyze the clinical characteristics and curative effect of multi-disciplinary collaboration(MDT)dental extraction patients in Beijing Puren Hospital,and provide reference for the allocation of medical resources.Methods The clinical data of 230 patients admitted from January 2019 to January 2020 were reviewed,and the general condition,clinic features and the completion of tooth extraction were analyzed.Results The average age of the survey population(70.1±12.4)years old,among which 81.7%were over 60 years old,mainly elderly people.Periodontal disease,odontogenic infection and irreparable crown and root were the main causes of tooth extraction.The main systemic complications were hypertension,coronary heart disease,diabetes and arrhythmia,accounting for 83.6%.40%of patients suffer from more than two complications,which is complex diverse and risky.The completion rate of MDT tooth extraction was 90.9%,55.9%of outpatients within 3 days,95.2%of outpatients and 81%of inpatients within 1 week,which was relatively safe and convenient.Conclusion MDT dental extraction in Puren Hospital has a good clinical effect,so it is necessary to give full play to the advantages of general hospitals,further rationally allocate the dental surgery resources based on MDT dental extraction,and actively develop the specialty characteristics to meet the medical needs of the aging society.
