沙库巴曲缬沙坦治疗慢性心力衰竭的效果观察

Observation of therapeutic effect of sacubitril-valsartan on chronic heart failure

目的探讨沙库巴曲缬沙坦治疗慢性心力衰竭(CHF)的效果。方法抽取2021年2月至2022年2月焦作煤业(集团)有限责任公司中央医院收治的87例CHF患者进行研究,根据入组先后顺序分为对照组与观察组,两组均采用常规治疗,对照组43例采用地高辛治疗,观察组44例在对照组基础上采用沙库巴曲缬沙坦治疗。比较两组治疗效果、心功能、肾功能及不良反应发生情况。结果观察组总有效率(95.45%,42/44)高于对照组(79.07%,34/43),P<0.05。治疗前,两组心功能指标、肾功能指标比较差异未见统计学意义(P>0.05);治疗后4周,观察组左室舒张末期内径、左室收缩末期内径小于对照组,左室射血分数、心脏指数高于对照组,血肌酐、尿素氮、尿蛋白排泄率低于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较差异未见统计学意义(P>0.05)。结论沙库巴曲缬沙坦治疗CHF,可进一步提高预后,更好地改善患者的心功能、肾功能,且不会明显增加不良反应,安全性好。

Objective To investigate the efficacy of sacubitril-valsartan in the treatment of chronic heart failure(CHF).Methods A total of 87 CHF patients treated in Central Hospital of Jiaozuo Coal Industry(Group)Co.,Ltd.from February 2021 to February 2022 were selected for the research.According to the order of enrollment,they were divided into control group and observation group.Both groups were treated by conventional treatment,43 cases in the control group were treated with digoxin,and 44 cases in the observation group were treated with sacubitril valsartan based on the treatment of the control group.The efficacy,cardiac function,renal function and adverse reactions of the two groups were compared.Results The total effective rate of the observation group(95.45%,42/44)was higher than that of the control group(79.07%,34/43),P<0.05.Before treatment,there was no significant difference in cardiac function indexes and renal function indexes between the two groups(P>0.05);4 weeks after treatment,the left ventricular end-diastolic diameter and left ventricular end-systolic diameter of the observation group were smaller than those of the control group,the left ventricular ejection fraction and cardiac index were higher than those of the control group,while the excretion rates of serum creatinine,urea nitrogen and urine protein were lower than those of the control group,and the difference was significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Sacubitril-valsartan in the treatment of CHF can further promote the prognosis,and better improve the patients’cardiac function and renal function,without significant increase in adverse reactions but good safety.
