基于安全-支持-合作过渡期护理模式的早产儿护理方案构建研究

Constructing the nursing scheme for preterm infants based on the secure-support-collaborate transitional nursing model

目的:基于“安全-支持-合作”(secure-encourage-collaborate,SEC)过渡期护理模式构建早产儿护理方案,旨在为临床护士实施更有针对性护理干预措施提供参考。方法:以“过渡期护理模式”为理论指导确定一级条目为转出前、转出中、转出后;基于“SEC理论”,确定二级条目为安全、支持、合作,通过文献回顾法初步拟定基于安全-支持-合作过渡期护理模式的早产儿护理方案,采用专家函询对早产儿护理方案的相关内容进行函询。结果:两轮专家咨询函有效回收率均为100%(15/15),专家权威系数分别为0.870、0.890。第2轮专家函询中29个条目重要性评分的算术均数在4.46~5.00分之间,标准差在0.00~0.81之间,满分比为0.25~1.00。两轮函询专家意见的肯德尔协调系数分别为0.224、0.165(P<0.01)。最终确定早产儿护理方案为一级条目3个、二级条目9个、三级条目29个。结论:基于安全-支持-合作过渡期护理模式构建的早产儿护理方案具有较高的科学性、可靠性,可用于规范早产儿过渡期护理实践。

Objective:To construct a nursing program for premature infants based on the"secure-support-collaborate"transitional nursing model,aiming to provide reference for clinical nurses to implement more targeted nursing interventions,so as to improve the quality of transitional nursing for premature infants.Methods:The"transitional nursing model"was used as the theoretical guidance to determine the first-level items including before transfer out,transfer out of the department and after transfer out.The"SEC theory"was used to determine the second-level items including secure,support and collaborate.A nursing plan for premature infants based on the secure-support-collaborate transitional nursing model was preliminarily formulated by consulting literature,and relevant content of the nursing plan for premature infants was consulted through expert consultation.Results:The effective recovery rates of two rounds of expert consultation were 100%(15/15),respectively,and the authority coefficients were 0.870 and 0.890,respectively.The mean of the importance scores of the 29 items in the second round of expert consultation was 4.46~5.00 points with the standard deviation ranging 0.00~0.81,and the full score ratio was 0.25~1.00.The Kendall coordination coefficients of the two rounds of consultation of experts were 0.224 and 0.165,respectively(P<0.01).The final care plan for premature infants contains 3 first-level items including 9 second-level items,and 29 third-level items.Conclusion:The nursing plan for premature infants based on the secure-support-collaborate transitional nursing model is highly scientific and reliable.It can be used to standardize the transitional nursing practice of premature infants in the NICU,and can provide objective indicators for the transition of premature infants and provide clinical support.
