基于CPET的T2DM伴心脏自主神经病变的心肺适能评估

Features of cardiopulmonary function in patients with type 2 diabetes mellitus and cardiac autonomic neuropathy based on CPET

目的:利用心肺运动试验(CPET)评估2型糖尿病心脏自主神经病变患者(DCAN)心肺适能。方法:筛选我院行CPET检查的DCAN 82例为观察组,同期行CPET检查的健康者78例为对照组。比较两组心肺适能核心指标。结果:观察组VO_(2)peak、AT、METpeak、VO_(2)/HRpeak、WRpeak、运动峰值心率、恢复心率、峰值收缩压、恢复期3min收缩压低于对照组,差异有统计学意义(P<0.05);VE/VCO_(2)、恢复期3min舒张压高于对照组,差异有统计学意义(P<0.05)。结论:DCAN存在心肺适能下降,CPET可精准评估该类患者心肺适能,为下一步制定个体化运动处方奠定基础。

Objective:To evaluate cardiorespiratory fitness in patients with type 2 diabetes mellitus and cardiac autonomic neuropathy(DCAN)by cardiopulmonary exercise test(CPET).Methods:82 patients with DCAN who underwent CPET in our hospital were selected as the observation group,and 78 healthy people who underwent CPET at the same time were selected as the control group.Compare the core indexes of cardiopulmonary fitness between the two groups.Results:The VO_(2)peak,AT,METpeak,VO_(2)/HRpeak,WRpeak,the peak heart rate,heart rate recovery,the peak systolic blood pressure and 3-minute systolic blood pressure during exercise recovery in the observation group were lower than the core indicators in the control group(P<0.05).VE/VCO_(2) and 3-minute diastolic blood pressure during exercise recovery was higher than that in the control group(P<0.05).Conclusion:Patients with DCAN have decreased cardiorespiratory fitness and decreased recovery of heart rate and blood pressure after exercise.CPET can accurately evaluate the cardiopulmonary function of patients and lay a foundation for formulating corresponding individualized exercise prescriptions in the next step.
