摘要
目的 探讨超低频经颅磁刺激对老年脑梗死后抑郁(PCID)的效果及对生活质量和脑涨落图仪参数的影响。方法 选取石家庄市第八医院收治的100例老年PCID患者作为研究对象,经随机数字表法分为观察组(50例)和对照组(50例)。对照组予以单纯药物治疗,观察组在此基础上予以超低频经颅磁刺激治疗。比较2组临床疗效及治疗前后抑郁情况、神经功能、生活质量、脑涨落图[去甲肾上腺素(NE)、多巴胺(DA)功率值]变化及不良反应。结果 观察组治疗总有效率为90.00%高于对照组74.00%(P<0.05);观察组治疗后汉密尔顿抑郁量表(HAMD)评分、美国国立卫生研究院卒中量表评分低于对照组(P<0.05);观察组治疗后物质生活状态、社会功能、心理功能、躯体功能等维度评分高于对照组(P<0.05);观察组治疗后大脑NE、DA功率高于对照组(P<0.05);2组不良反应比较,差异无统计学意义(P>0.05)。结论 超低频经颅磁刺激能提高PCID的治疗效果,有效改善患者抑郁程度,促进神经功能恢复,提高生活质量,调节脑神经递质,不增加不良反应,安全性高。
Objective To explore the curative effect of ultra-low frequency transcranial magnetic stimulation on post-cerebral infarction depression(PCID)in the elderly and its influences on quality of life and encephalofluctuograph parameters.Methods A total of 100 elderly patients with PCID in the hospital were selected as the research objects.According to random number table method,they were divided into observation group(n=50)and control group(n=50).The control group was given simple drug treatment,while observation group was additionally given ultra-low frequency transcranial magnetic stimulation.The clinical curative effect,depression,nerve function,quality of life,encephalofluctuograph parameters[norepinephrine(NE),dopamine(DA)]and adverse reactions before and after treatment were compared between the two groups.Results The total response rate of treatment in observation group was higher than that in control group(90.00%vs.74.00%,P<0.05).After treatment,HAMD and NIHSS scores in observation group were lower than those in control group(P<0.05),scores of living state,social function,psychological function and physical function were higher than those in control group(P<0.05),and power values of NE and DA were greater than those in control group(P<0.05).The difference in adverse reactions between the two groups was not statistically significant(P>0.05).Conclusions Ultra-low frequency transcranial magnetic stimulation can improve curative effect on PCID,effectively relieve depression,promote the recovery of nerve function,improve quality of life,adjust encephalofluctuograph parameters with less adverse reactions and high safety.
