无痛分娩与自然分娩的临床对比研究

Clinical comparative study of painless labor and natural labor

目的研究无痛分娩与自然分娩的临床结局。方法选取160例足月分娩产妇,根据电脑流水编号将产妇随机分为参照组(自然分娩)与试验组(无痛分娩),每组80例。比较两组产妇产程时间、镇痛效果、母婴结局及泌乳情况。结果试验组第一产程、总产程均短于参照组,差异具有统计意义(P<0.05),两组在第二产程、第三产程比较,差异无统计学意义(P>0.05)。试验组产妇镇痛优良率高达97.5%,高于参照组的80.0%,差异有统计学意义(P<0.05)。试验组新生儿评分高于参照组,产后出血量少于参照组,剖宫产率低于参照组(P<0.05)。试验组开始泌乳时间短于参照组,且24 h哺乳次数多于参照组(P<0.05)。结论在产妇分娩过程中,无痛分娩的应用效果更好,能有效减轻疼痛,提高自然分娩率,同时降低产后出血量,且更利于新生儿的健康,具有临床推广价值。

Objective To study the clinical outcome of painless labor and natural labor.Methods 160 full-term parturients were selected and randomly divided into the reference group(natural delivery)and the experimental group(painless delivery)according to the computer numbering,with 80 cases in each group.Labor time,analgesic effect,maternal and infant outcomes and lactation were compared between the two groups.Results The first and total stages of labor in the experimental group were shorter than those in the reference group,the difference was statistically significant(P<0.05),but there was no significant difference between the two groups in the second and third stages of labor(P>0.05).The rate of analgesia in experimental group was 97.5%,higher than that in reference group(80.0%),and the difference was statistically significant(P<0.05).Neonatal score of experimental group was higher than that of reference group,postpartum blood loss was lower than that of reference group,cesarean section rate was lower than that of reference group(P<0.05).The first lactation time of experimental group was shorter than that of control group,and the number of lactation at 24 h was longer than that of control group(P<0.05).Conclusion In the process of maternal delivery,painless delivery has a better application effect,which can effectively reduce pain,improve the rate of natural delivery,reduce postpartum blood loss at the same time,and is more conducive to the health of newborns,with clinical promotion value.
