急性非致残性缺血性脑血管事件高危人群CYP2C19基因多态性与氯吡格雷抵抗的关系

Relationship between the polymorphism of CYP2C19 gene and clopidogrel resistance in high-risk patients with acute non-disabling cerebrovascular events

目的探讨急性非致残性缺血性脑血管事件高危人群CYP2C19基因多态性与氯吡格雷抵抗的关系。方法选取2020年1月至2021年3月就诊于九江市第一人民医院的160例急性非致残性缺血性脑血管事件高危人群作为研究对象,采用京因药物基因床旁检测系统来实现CYP2C19基因床边快速检测,依据CYP2C19基因位点分为快代谢组64例、中间代谢组76例、慢代谢组20例。比较不同代谢基因组患者的血小板ADP抑制率及氯吡格雷抵抗情况的差异。结果不同代谢基因组中氯吡格雷抵抗率情况比较,差异无统计学意义(P>0.05);不同代谢基因组血小板ADP抑制率比较,差异无统计学意义(P>0.05)。结论对于服用氯吡格雷的急性非致残性缺血性脑血管事件高危人群的患者,CYP2C19基因多态性与氯吡格雷抵抗尚不能确定有确切关联。

Objective To investigate the relationship between CYP2C19 gene polymorphism and clopidogrel resistance in people at high risk of acute non-disabling ischemic cerebrovascular events.Methods A total of 160 patients with high risk of acute non-disabling ischemic cerebrovascular events were enrolled in the Jiujiang First People's Hospital from January 2020 to March 2021.The Jingyin drug gene bedside detection system was used to detect the CYP2C19 gene rapidly.According to the CYP2C19 gene locus,the cases were classified into 64 cases of fast metabolic group,76 cases of intermediate metabolic group,and 20 cases of slow metabolic group.The inhibition rate of platelet ADP and clopidogrel resistance in patients with different metabolic genomes groups were compared.Results There was no significant difference in clopidogrel resistance rate among different metabolic genomes groups(P>0.05).There was no significant difference in platelet ADP inhibition rate among different metabolic genomes groups(P>0.05).Conclusion In the patients at high risk of acute nondisabling ischemic cerebrovascular events,the association between CYP2C19 gene polymorphism and clopidogrel resistance can not be established yet.
