疫情背景下中医主诉挖掘的解析

Analysis on Mining of Traditional Chinese Medicine Chief Complaints under the Background of Epidemic Situation

新型冠状病毒肺炎(Corona virus disease 2019,COVID-19)符合中医“瘟疫”的描述,中医作为一门实用性很强的学科门类,其问诊的核心内容均是围绕主诉来展开,所有的医疗行为也建立在主诉的基础上。结合当前常见的疫情表现,此文梳理了中医诊断临床思维之起始——“主诉”的相应变化,如主诉定义中“患者”概念的外延;疫情背景下主诉分类倾向性的变化;疫情中中医主诉横向与纵向挖掘的要点;联系疫情常见主症,初步总结了围绕主诉可重点选用的辨证方法等。文章旨在通过对主诉挖掘的解析,帮助临床早期诊断疾病、评估病情、制定诊疗方案,同时相信随着人类对疾病的进一步了解,对诊治经验的进一步积累,未来围绕主诉也能获得更详实、清晰的临床认识。

The COVID-19 is consistent with the description of“plague”in traditional Chinese medicine.As a highly practical subject category,the core content of TCM is centered around the main complaint,and all medical acts are based on the chief complaint.Combined with the current common epidemic manifestations,we sorted out the corresponding changes of“main complaint”,the beginning of clinical thinking of TCM diagnosis,such as the extension of the concept of“patient”in the definition of main complaint,the change of classification tendency of main complaints under the background of epidemic situation,the main points of horizontal and vertical excavation of TCM complaints in the epidemic situation.Based on the common main symptoms of the epidemic situation,this paper preliminarily summarizes the key dialectical methods that can be selected around the main complaint,etc.This paper aims to help clinical early diagnosis of disease,evaluation of disease condition and formulation of diagnosis and treatment plan through the analysis of main complaint mining.At the same time,it is believed that with the further understanding of disease and the further accumulation of diagnosis and treatment experience,more detailed and clear clinical understanding can be obtained around main complaint in the future.
