“医院-社区-家庭”三元联动延续护理模式在老年髋关节置换术后患者康复中的应用价值

Application value of"hospital-community-family"three-element linkage continuous nursing model in rehabilitation of elderly patients after hip replacement

目的分析“医院-社区-家庭”三元联动延续护理模式在老年髋关节置换术后患者康复中的应用价值。方法选取我院2020年1月至12月收治的90例老年髋关节置换术患者为研究对象,根据护理方案差异将其分为常规组(n=45,院内康复护理)和观察组(n=45,“医院-社区-家庭”三元联动延续护理模式)。比较两组的干预效果。结果观察组术后康复期间的关节脱位、泌尿系统感染、下肢静脉血栓及肺部感染发生率均低于常规组(P<0.05)。观察组的康复锻炼总依从率高于常规组(P<0.05)。出院3个月,观察组的Harris、简明健康状况调查量表(SF-36)及Barthel指数评分均显著高于常规组(P<0.05)。结论在老年髋关节置换术后患者康复中实施“医院-社区-家庭”三元联动延续护理模式可取得理想的护理效果。

Objective To analyze the application value of"hospital-community-family"three-element linkage continuous nursing model in rehabilitation of elderly patients after hip replacement.Methods Ninety elderly patients undergoing hip replacement admitted in our hospital from January 2020 to December were selected as the study objects and divided into conventional group(n=45,in-hospital rehabilitation nursing)and observation group(n=45,"hospital-community-family"three-element linkage continuous nursing model)according to the differences in nursing plans.The intervention effects of the two groups were compared.Results The incidences of joint dislocation,urinary system infection,lower extremity venous thrombosis and pulmonary infection in the observation group during postoperative rehabilitation were lower than those in the conventional group(P<0.05).The total compliance rate of rehabilitation exercise in the observation group was higher than that in the conventional group(P<0.05).Three months after discharge,the scores of Harris,Short-Form-36 Health Survey(SF-36)and Barthel index in the observation group were significantly higher than those in the conventional group(P<0.05).Conclusion The"hospital-community-family"three-element linkage continuous nursing model applied in the rehabilitation of elderly patients after hip replacement can achieve ideal nursing effect.
