以需求为导向的健康教育结合支持性心理疗法对老年2型糖尿病患者自我管理能力、心理状态及血糖控制情况的影响 被引量:7

Effects of demand-oriented health education combined with supportive psychotherapy on self-management ability,psychological state and blood glucose control in elderly patients with type 2 diabetes mellitus

摘要 目的探讨以需求为导向的健康教育结合支持性心理疗法在老年2型糖尿病(T2DM)患者中的应用价值。方法选择2020年1月至2021年1月我院收治的88例老年T2DM患者为研究对象,按随机数字表法将其分为对照组和观察组,各44例。对照组给予常规护理,观察组在对照组基础上给予以需求为导向的健康教育结合支持性心理疗法。比较两组的干预效果。结果干预后,观察组的血糖管理、饮食管理、运动管理及用药管理评分高于对照组(P<0.05)。干预后,观察组的汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)评分低于对照组(P<0.05)。干预后,观察组的日间血糖平均绝对差(MODD)、日内平均血糖漂移幅度(MAGE)水平低于对照组,血糖控制达标率高于对照组(P<0.05)。结论以需求为导向的健康教育结合支持性心理疗法可提高老年T2DM患者自我管理能力,调节心理状态,提高血糖控制效果。 Objective To explore the application value of demand-oriented health education combined with supportive psychotherapy in elderly patients with type 2 diabetes mellitus(T2DM).Methods A total of 88 elderly patients with T2DM admitted in our hospital from January 2020 to January 2021 were selected as the research objects and divided into control group and observation group according to the random number table method,with 44 cases in each group.The control group was given routine nursing,and the observation group was given demand-oriented health education combined with supportive psychotherapy on the basis of the control group.The intervention effects of the two groups were compared.Results After intervention,the scores of blood glucose management,diet management,exercise management and medication management in the observation group were higher than those in the control group(P<0.05).After intervention,the scores of Hamilton Depression Scale(HAMD)and Hamilton Anxiety Scale(HAMA)in the observation group were lower than those in the control group(P<0.05).After intervention,the levels of absolute means of daily differences(MODD)and mean amplitude of glucose excursions(MAGE)in the observation group were lower than those in the control group,and the standard-reaching rate of blood glucose control was higher than that in the control group(P<0.05).Conclusion The demand-oriented health education combined with supportive psychotherapy can improve the self-management ability of elderly patients with T2DM,regulate the psychological state,and improve the effect of blood glucose control.
