高频彩超诊断下肢深静脉血栓的临床应用研究

Clinical application of high frequency color ultrasound in diagnosis of lower limb deep venous thrombosis

目的:探讨高频彩超在诊断下肢深静脉血栓(deep venous thrombosis,DVT)中的价值。方法:选取2020年11月—2021年12月期间日照市中医医院收治的85例下肢深静脉血栓患者作为病例组,纳入同期于日照市中医医院接受健康体检的80名体检者作为对照组。两组均接受高频彩超检查,分析高频彩超对病例组和对照组静脉反流、管腔内不均质回声和内径扩张的检出率以及高频超声对不同类型DVT的检出情况。结果:病例组静脉反流检出率12.94%(11/85)、管腔内不均质回声检出率77.65%(66/85)以及内径扩张检出率68.24%(58/85)均高于对照组的2.25%(2/80)、7.50%(6/80)以及12.50%(10/80),差异有统计学意义(P<0.05);根据侧别,高频超声检查出单侧占61.18%,双侧占38.82%;根据部位高频超声检查出,单纯性DVT(34.11%)、合并小腿肌间静脉血栓(14.17%)、单纯肌间静脉血栓(34.11%)、浅表静脉血栓(5.88%);根据血栓类型,周围性占23.53%、中央型52.94%、混合型23.53%;根据血栓特征高频超声检查出慢性血栓(60.00%)、急性血栓(22.35%)、亚急性血栓(17.65%)。结论:高频彩超对下肢DVT具有较好的诊断价值。

Objective To explore the value of high-frequency color Doppler ultrasound in the diagnosis of deep venous thrombosis(DVT)of lower limbs.Methods 85 patients with deep venous thrombosis of lower limbs admitted to Rizhao Hospital of Traditional Chinese Medicine from November 2020 to December 2021 were selected,and 80 cases who received physical examination in Rizhao Hospital of Traditional Chinese Medicine during the same period were included as the control group.Both groups were examined by high-frequency color Doppler ultrasound,and the detection rate of venous reflux,heterogeneous echo and inner diameter expansion in the lumen and the detection of different types of DVT by high-frequency color Doppler ultrasound were analyzed.Results In the case group,the detection rate of venous reflux was 12.94%(11/85),the detection rate of waslumen heterogeneity echo was 77.65%(66/85),and the detection rate of inner diameter dilation was 68.24%(58/85),which was higher than that of the control group 2.25%(2/80),7.50%(6/80),and 12.50%(10/80).According to the lateral classification,high-frequency ultrasound showed unilateral(61.18%)and bilateral(38.82%);According to the examination of high-frequency ultrasound,simple DVT(34.11%),intermuscular venous thrombosis(14.17%),simple intermuscular venous thrombosis(31.76%)and superficial venous thrombosis(5.88%)were found.According to the type of thrombus,peripheral type(23.53%),central type(52.94%)and mixed type(23.53%);According to the characteristics of thrombus,chronic thrombus(60.00%),acute thrombus(22.35%)and subacute thrombus(17.65%)were detected by high-frequency ultrasound.Conclusion High-frequency color Doppler ultrasound has good diagnostic value for DVT of lower limbs.
