DCE-MRI定量参数对肺部良恶性病变的诊断价值

摘要 目的:探讨动态增强磁共振成像(dynamic contrast-enhanced magnetic resonance imaging,DCE-MRI)定量参数对肺部良恶性病变的诊断价值。方法:选取烟台市龙口荣军医院2021年1月—2022年8月收治的90例肺部良恶性病变患者,根据穿刺活检病理诊断结果,将患者分为良性病变组(41例)和恶性病变组(49例)。分析DCE-MRI对肺部良恶性病变的鉴别诊断结果,比较良性病变组和恶性病变组的DCE-MRI定量参数,绘制ROC曲线并分析DCE-MRI定量参数对肺部良恶性病变的诊断价值。结果:90例肺部良恶性病变患者经DCEMRI检查共检出46例阳性,44例阴性;恶性病变组K^(trans)、K_(ep)显著高于良性病变组(P<0.01),两组TTP、MAX_(slope)、MAX_(conc)、V_(p)、V_(e)差异无统计学意义(P>0.05);ROC曲线显示,K^(trans)诊断肺部良恶性病变AUC为0.930,截断值为0.24,K_(ep)诊断肺部良恶性病变AUC为0.853,截断值为0.86。结论:DCE-MRI定量参数K^(trans)、K_(ep)在肺癌良恶性病变患者中具有较高的诊断效能。 Objective To investigate the diagnostic value of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)quantitative parameters in benign and malignant lung lesions.Methods 90 patients with benign and malignant lung lesions admitted to Yantai Longkou Rongjun Hospital from January 2021 to August 2022 were selected and divided into benign lesion group(41 cases)and malignant lesion group(49 cases)according to the pathological diagnosis results of puncture biopsy.Differential diagnosis results of DCE-MRI for benign and malignant lung lesions were analyzed,quantitative parameters of DCE-MRI in benign and malignant lung lesions were compared,the ROC curve was drawn,and diagnostic value of quantitative parameters of DCE-MRI for benign and malignant lung lesions was analyzed.Results A total of 46 positive and 44 negative cases were detected by DCE-MRI in 90 patients with benign and malignant pulmonary lesions.K^(trans)and K_(ep)in malignant lesion group were significantly higher than those in benign lesion group(P<0.01),but TTP,MAX_(slope),MAX_(conc),V_(p)and V_(e)were not significantly different between the two groups(P>0.05).The ROC curve showed that,the AUC of K^(trans)for the diagnosis of benign and malignant lung lesions was 0.930,and the cut-off value was 0.24;the AUC of K_(ep)for the diagnosis of benign and malignant lung lesions was 0.853,and the cut-off value was 0.86.Conclusion DCE-MRI quantitative parameters K^(trans)and K_(ep)have high diagnostic efficacy in patients with benign and malignant lung cancer lesions.
