白噪声疗法干预对早产儿神经行为发育及操作性疼痛的影响 被引量:1

摘要 目的:探讨白噪声疗法干预对早产儿神经行为发育及操作性疼痛的影响。方法:选取2020年3月—2022年2月郑州大学附属儿童医院收治的74例早产儿作为研究对象,采用随机数表法分为试验组与对照组,每组各37例。对照组实施常规护理干预,试验组在常规护理基础上实施白噪声疗法干预。比较两组早产儿操作性疼痛[修订版早产儿疼痛评估量表(PIPP-R)评分]、活动及睡眠监测、神经行为发育[中国20项新生儿行为神经测定表(NBNA)评分]情况。结果:试验组早产儿穿刺操作时、穿刺后1 min、穿刺后5 min的PIPP-R评分均低于对照组,差异有统计学意义(t=-5.311、-3.728、-4.924,P<0.05)。干预后,试验组早产儿活动度低于对照组,睡眠时间多于对照组,差异有统计学意义(t=6.097、8.170,P<0.05)。干预后,试验组NBNA量表中各维度评分及总分均高于对照组,差异有统计学意义(t=5.008、5.511、6.151、7.876、8.278、6.260,P<0.05)。结论:白噪声疗法干预能够减轻早产儿操作性疼痛程度,减少早产儿的活动度,增加其睡眠时间,促进早产儿神经行为发育。 Objective:To explore the effect of white noise therapy on the neurobehavioral development and operative pain of premature infants.Methods:74 cases of premature infants admitted to the hospital from March 2020 to February 2022 were selected as the study subjects and were divided into a test group and a control group using the random number table method,with 37 cases in each group.The control group implemented conventional nursing interventions,and the experimental group implemented white noise therapy interventions on the basis of conventional nursing.The operational pain(Revised Preterm Infant Pain Assessment Scale[PIPP-R]scores),activity and sleep monitoring,and neurobehavioral development(Chinese 20-item Neonatal Behavioral Neu⁃rological Assay Scale[NBNA]scores)of preterm infants were compared between the two groups.Results:The PIPP-R scores at the time of puncture operation,1 min after puncture,and 5 min after puncture were lower in the test group of preterm infants than in the control group,and the differences were statistically significant(t=-5.311,-3.728,-4.924,P<0.05).After the intervention,preterm infants in the test group were less active and slept more than the control group,with statistically significant differences(t=6.097,8.170,P<0.05).After the intervention,all dimensional scores and total scores in the NBNA scale were higher in the test group than in the control group,and the differences were statistically significant(t=5.008,5.511,6.151,7.876,8.278,6.260,P<0.05).Conclusion:White noise therapy interventions can reduce the level of operant pain,reduce the mobility of preterm infants,increase their sleep time,and promote neurobehavioral development in preterm infants.
