六步癌症告知模型应用于胃癌根治术后患者的效果及对生活质量的影响

Effectiveness of Six-step Cancer Informing Model Applied to Patients after Radical Gastric Cancer Surgery and its Effect on Quality of Life

目的:探讨六步癌症告知模型(SPIKES)应用于胃癌根治术后患者的效果及对生活质量的影响。方法:选取2020年1月—2021年5月郑州市第三人民医院收治的100例胃癌患者作为研究对象。采用随机数表法将其分为对照组与观察组,每组各50例。对照组采用常规告知,观察组在常规告知基础上采用SPIKES告知。比较两组患者的心理状况[汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)评分]、临床疗效(肛门排气时间、排便时间、平均住院时间)、生活质量评分。结果:干预后,观察组HAMA、HAMD评分均低于对照组,差异有统计学意义(t=10.659、17.082,P<0.05)。观察组肛门排气时间和排便时间均快于对照组,平均住院时间短于对照组,差异有统计学意义(t=7.113、9.395、4.693,P<0.05)。干预后,两组患者生活质量各维度评分明显高于干预前,且观察组明显高于对照组,差异有统计学意义(P<0.05)。结论:采用SPIKES可以改善患者对胃癌根治术后所产生的焦虑、抑郁等不良情绪,能够有效地加快患者术后的肛门排气时间、排便时间,缩短患者的住院时间,从而提高患者的生活质量。

Objective:To investigate the effect of the six-step cancer informing model(SPIKES)applied to patients after radi⁃cal gastric cancer surgery and its impact on quality of life.Methods:From January 2020 to May 2021,100 patients with gastric cancer admitted to the hospital were selected as the study subjects,and they were divided into control and observation groups using the random number table method,with 50 cases in each group.The control group was informed conventionally and the obser⁃vation group was informed by SPIKES on top of the conventional information.The psychological status(Hamilton Anxiety Inventory[HAMA]and Hamilton Depression Inventory[HAMD]scores),clinical outcomes(time to anal discharge,time to defecation,mean length of stay),and quality of life scores of the two groups were compared.Results:After the intervention,the HAMA and HAMD scores of patients in the observation group were lower than those in the control group,and the differences were statistically signifi⁃cant(t=10.659,17.082,P<0.05).The time to anal evacuation and defecation were faster in the observation group than in the con⁃trol group,and the mean hospital stay was shorter than in the control group,with statistically significant differences(t=7.113,9.395,4.693,P<0.05).After the intervention,the scores of each dimension of quality of life were significantly higher in both groups than before the intervention,and the observation group was significantly higher than the control group,and the difference was statistically significant(P<0.05).Conclusion:The adoption of SPIKES model can improve patients’anxiety and depression adverse emotions after radical gastric cancer surgery,and can effectively accelerate patients’postoperative anal venting time and defecation time,shorten patients’hospitalization time,and thus improve patients’quality of life.
