彩色多普勒超声测量子宫弓状动脉血流参数对孕期前置胎盘合并植入的评估价值

Evaluation Value of Uterine Arcuate Artery Blood Flow Parameters Measured by Color Doppler Ultrasound in Placenta Previa with Implantation During Pregnancy

目的分析彩色多普勒超声测量子宫弓状动脉血流参数对孕期前置胎盘合并植入的评估价值。方法78例前置胎盘孕妇均接受腹部彩色多普勒超声检查并测量子宫弓状动脉血流相关参数。随访至孕妇终止妊娠,统计前置胎盘合并植入发生情况,并评估子宫弓状动脉血流参数对前置胎盘合并植入的预测价值。结果78例前置胎盘孕妇中,有21例合并胎盘植入。植入组的PSV高于未植入组,RI、PI均低于未植入组(P<0.05)。ROC曲线显示,PSV、RI、PI单独及联合预测前置胎盘合并植入的AUC均>0.80,且以联合预测效能最好。结论彩色多普勒超声测量子宫弓状动脉血流参数对前置胎盘合并植入具有较高的预测价值。

Objective To analyze the evaluation value of uterine arcuate artery blood flow parameters measured by color Doppler ultrasound in placenta previa with implantation during pregnancy.Methods All 78 pregnant women with placenta previa received abdominal color Doppler ultrasound examination and measurement of uterine arcuate artery blood flow related parameters.The pregnant women were followed up until the termination of pregnancy,the occurrence of placenta previa with implantation was counted,and the predictive value of uterine arcuate artery blood flow parameters for placenta previa with implantation was evaluated.Results Of the 78 pregnant women with placenta previa,21 had placenta implantation.The PSV of the implanted group was higher than that of the non-implanted group,and the RI and PI were lower than those of the non-implanted group(P<0.05).ROC curve showed that the AUC of single prediction and combined prediction of PSV,RI and PI for placenta previa with implantation were higher than 0.80,and the combined prediction had the best efficiency.Conclusions Uterine arcuate artery blood flow parameters measured by color Doppler ultrasound have high predictive value for placenta previa with implantation.
