不同剂量左西孟旦对老年重症心力衰竭患者心功能及血流动力学的影响

Impact of Different Doses of Levosimendan on Cardiac Function and Hemodynamics of Elderly Patients with Severe Heart Failure

目的探讨不同剂量左西孟旦对老年重症心力衰竭患者心功能及血流动力学的影响。方法选取2019年11月至2022年11月我院收治的80例老年重症心力衰竭患者,随机分为两组各40例。对照组给予小剂量左西孟旦治疗,观察组给予大剂量左西孟旦治疗。比较两组的心功能、血流动力学指标。结果治疗结束时,两组的m MRC评分均较治疗前降低,HR、LVEF均升高(P<0.05);且观察组的mMRC评分低于对照组,HR、LVEF均高于对照组(P<0.05)。治疗结束时,两组的PAMP均较治疗前降低,CI均升高(P<0.05);且观察组的PAMP低于对照组,CI高于对照组(P<0.05)。结论左西孟旦治疗老年重症心力衰竭可减轻患者呼吸困难症状,改善心功能及血流动力学状态;大剂量左西孟旦治疗效果更明显,但可增加心律失常风险。

Objective To explore the impact of different doses of levosimendan on cardiac function and hemodynamics of elderly patients with severe heart failure.Methods 80 cases of elderly patients with severe heart failure admitted to our hospital from November 2019 to November 2022 were selected and randomly divided into two groups,with 40 cases in each group.The control group was treated with low-dose levosimendan,and the observation group was treated with high-dose levosimendan.The cardiac function and hemodynamic indicators were compared between the two groups.Results At the end of treatment,the mMRC scores of the two groups were lower than those before treatment,and HR and LVEF were higher than those before treatment(P<0.05);The mMRC score of the observation group was lower than that of the control group,and HR and LVEF were higher than those of the control group(P<0.05).At the end of treatment,PAMP of the two groups were lower than those before treatment,and CI were higher than those before treatment(P<0.05);PAMP of the observation group was lower than that of the control group,and CI was higher than that of the control group(P<0.05).Conclusions Levosimendan in the treatment of elderly severe heart failure can alleviate the symptoms of dyspnea,and improve cardiac function and hemodynamic status of patients.High-dose levosimendan is more effective,but it can increase the risk of arrhythmia.
