急性期、缓解期RRMS患者静息态功能网络连接的fMRI研究

摘要 目的运用功能网络连接方法探讨复发缓解型MS(relapsing remitting MS,RRMS)患者急性期、缓解期的大尺度静息态功能网络间的延迟相关性。方法选取15例RRMS急性期患者(为急性期组)、23例缓解期患者(缓解期组)及23例年龄、性别和教育匹配的健康者对照。受试者接受静息态功能成像扫描及临床量表评分,在Matlab平台上采用高模型阶数的组间独立成分分析算法分离出独立分量,基于最大空间相关模板匹配算法识别出8个认知相关RSNs,应用FNC工具箱执行Lag-shift算法,计算RSNs间在时间上的最大延时相关系数,比较急性期RRMS患者与健康对照组、缓解期RRMS患者与健康对照组功能网络连接差异。结果RRMS患者急性期组与缓解期组扩展残疾量表得分存在统计学差异。与健康对照组比较,RRMS患者急性期组前突显网络在时间上滞后于背侧默认网络;高级视觉网络滞后于右侧执行控制网络。缓解期组RRMS患者右侧执行控制网络滞后于前突显网络,背侧默认网络滞后于初级视觉网络、高级视觉网络。结论RRMS患者存在认知相关功能网络连接异常,背侧默认网络、右侧执行控制网络功能连接改变在缓解期组RRMS患者功能代偿机制中具有重要作用。 Objective Previous studies have shown that abnormalities in the internal network performance of brain function networks are related to cognitive impairment in relapsing remitting MS(RRMS).However,the functional network connections on cognitive-related resting-state networks(RSNs)in different disease stages are poorly understood.Methods Fifteen patients in acute phase,23 patients in remission phase,and 23 healthy controls matched for age,sex and education were recruited.Subjects received resting state functional imaging scans and clinical scale scores.The independent components were separated using a group independent component analysis algorithm of high model order on the Matlab platform,and 8 cognitive correlations were identified based on the maximum spatial correlation template matching algorithm RSNs.Using the FNC toolbox to executed the Lag-shift algorithm,we calculated the correlation coefficient of the maximum time delay between RSNs,and compared the functional network connection differences between RRMS patients in the acute phase and healthy controls,and remission RRMS patients and healthy controls.Results There was a statistically significant difference in extended disability scale scores between the acute and remission RRMS patient groups.Compared with the healthy control group,the front highlight network in the acute RRMS patient group lagged behind the dorsal default network in time;the advanced vision network lagged behind the right executive control network.And in patients with RRMS in remission,the right executive control network lagged behind the front highlight network,and the dorsal default network lagged behind the primary visual network and the advanced visual network.Conclusion Patients with RRMS have abnormal network connections for cognitive-related functions.The functional connection changes of the dorsal default network and the right executive control network may play an important role in the functional compensation mechanism of RRMS patients in remission.
