踝关节扭伤需手术干预的危险因素分析

Analysis of Risk Factors for Surgical Intervention of Ankle Ssprain

探讨踝关节扭伤需手术干预的危险因素。方法:通过收集我院门急诊2018年1月-2020年1月就诊的140例踝关节扭伤患者,根据扭伤后是否需手术干预,分为手术干预组和非手术干预组。比较2组的相关临床资料,通过Logistics回归模型分析踝关节扭伤手术干预的影响因素。结果:所有患者随访超过12个月,其中20例需手术干预。单因素分析下手术干预组和非手术干预组在身体质量指数(Body Mass In-dex,BMI)、糖尿病病史、吸烟史、扭伤次数、外踝瘀斑、使用支具固定等危险因素差异有统计学意义(P<0.05),通过多因素Logistics回归分析,BMI、扭伤次数、外踝瘀斑、使用支具固定是踝关节扭伤需手术干预的重要危险因素。结论:BMI、扭伤次数、外踝瘀斑及使用支具固定是踝关节扭伤需手术干预的重要危险因素,在初次踝关节扭伤时使用支具固定,一定程度上可以降低需手术治疗的风险。

Objective:To investigate the risk factors of ankle sprain requiring surgical intervention.Methods:A total of 140 patients with ankle sprain who visited the outpatient and emergency department in our hospital from Janu-ary 2018 to January 2020 were collected and divided into a surgical intervention group and a nonsurgical intervention group according to whether surgical intervention was required after the sprain.The clinical data of the two groups were compared,and the influencing factors of surgical intervention for ankle sprain were analyzed by logistic regres-sion model.Results:All patients were followed up for more than 12 months,and 20 of them required surgical inter-vention.Under analysis,the surgical intervention group and the nonsurgical intervention group had statistically signif-icant differences in risk factors such as body mass index(BMI),history of diabetes,smoking history,number of sprains,lateral malleolus ecchymosis,the difference was statistically significant(P<0.05).through multivariate lo-gistic regression analysis,BMI,the number of sprains,lateral ankle ecchymosis,and use of braces were important risk factors for ankle sprains requiring surgical intervention.Conclusion:BMI,number of sprains,lateral ankle ec-chymosis and use of braces are important risk factors for ankle sprains requiring surgical intervention.The use of braces in the initial ankle sprain can reduce the risk of surgical treatment to a certain extent.
