卡维地洛和美托洛尔治疗高血压的疗效及其对糖脂代谢影响的对比评价

Comparative Evaluation of the Efficacy of Carvedilol and Metoprolol in the Treatment of Hypertension and Their Effects on Glucolipid Metabolism

目的探究高血压患者在临床治疗过程中应用卡维地洛、美托洛尔两种药物的作用,并分析两者差异性。方法选取2019年11月—2021年5月贵阳市第一人民医院诊治的86例高血压患者,根据诊治时间将其分为对照组(使用美托洛尔治疗)和研究组(使用卡维地洛治疗),每组43例,分析并对比两组糖脂代谢指标、血压水平、治疗效果及不良反应发生情况。结果治疗后,研究组三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、空腹血糖(FPG)、餐后2 h血糖(2 hPG)、糖化血红蛋白(HbA1c)水平分别为(1.39±0.62)mmol/L、(5.53±0.94)mmol/L、(3.16±0.21)mmol/L、(1.91±0.41)mmol/L、(4.63±0.77)mmol/L、(9.80±1.26)mmol/L、(5.68±0.97)%,均优于对照组,差异有统计学意义(t=2.700、3.180、6.354、4.926、4.463、6.351、3.872,P<0.05);研究组治疗有效率(100.00%)高于对照组,差异有统计学意义(χ^(2)=5.600,P<0.05);两组舒张压、收缩压和不良反应发生率比较,差异无统计学意义(P>0.05)。结论应用美托洛尔或卡维地洛治疗高血压患者对于控制并稳定血压水平,缓解病情进展均有显著的效果,且不良反应发生率较低,但是在纠正糖脂代谢紊乱方面,卡维地洛的效果更佳,能够从整体上提高高血压患者的治疗效果,可有效改善预后。

Objective To investigate the effect of applying two drugs,carvedilol and metoprolol,in the clinical treat-ment of hypertensive patients only,and to analyze the difference between them.Methods 86 hypertensive patients treated in Guiyang First People's Hospital from November 2019 to May 2021 were selected,they were divided into a control group(treated with metoprolol)and a study group(treated with carvedilol)according to the time of diagnosis and treatment,with 43 cases in each group.The glycolipid metabolism indexes,blood pressure levels,treatment ef-fects and the occurrence of adverse effects were analyzed and compared between the two groups.Results After treat-ment,the study group of glycolipid metabolism indexes,including triglyceride(TG),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),fasting plasma glucose(FPG),2 h postprandial blood glucose(2 hPG),and glycated hemoglobin(HbA1c)levels were(1.39±0.62)mmol/L,(5.53±0.94)mmol/L,(3.16±0.21)mmol/L,(1.91±0.41)mmol/L,(4.63±0.77)mmol/L,(9.80±1.26)mmol/L,(5.68±0.97)%,respec-tively,which were all better than control group,the difference was statistically significant(t=2.700,3.180,6.354,4.926,4.463,6.351,3.872,P<0.05).The treatment efficiency of the study group was 100.00%,higher than that of the control group,the difference was statistically significant(χ^(2)=5.600,P<0.05).The diastolic and systolic BP levels and incidence of adverse effects in the two groups were compared,the difference was not statistically significant(P>0.05).Conclusion The treatment of hypertensive patients with metoprolol or carvedilol has a significant effect on controlling and stabilizing blood pressure levels and relieving the progression of the disease,and the incidence of adverse reac-tions is low.However,carvedilol was more effective in correcting disorders of glucolipid metabolism,which could im-prove the overall treatment effect of hypertensive patients and could effectively improve the prognosis.
