冷冻胚胎复苏移植中单囊胚移植质量与单胎出生体质量的关系

The relationship between the transfer quality of single blastocyst and the birthweight of singleton in frozen-embryo transfer

摘要 目的分析冷冻胚胎复苏移植(FET)中单囊胚移植质量与单胎出生体质量的相关性。方法选取2012年1月—2019年7月在厦门大学附属妇女儿童医院生殖医学科行冻融单囊胚移植的单胎活产病例379例为研究对象,回顾性分析其临床资料,根据囊胚质量分为4组:顶级(20例)、优质(73例)、一般(231例)及差(55例),测量单胎出生体质量。通过多元线性回归分析,研究胚胎质量与新生儿出生体质量之间的关系。结果与高质量的囊胚相比,低质量的囊胚平均出生体质量较低,相差114.73 g,差异无统计学意义(95%CI:-233.276~-53.652,P>0.05)。女方BMI值对孕周存在影响,差异有统计学意义(95%CI:-0.206~-0.055,P<0.05)。BMI越小早产的风险越大。结论复苏周期中不同质量的单囊胚移植对新生儿出生体质量无影响。 Objective To analyze the relationship between the embryo quality of a single transferred blastocyst and singleton birthweight in frozen-embryo transfer(FET)cycles.Methods This study involved 379 singleton live births born to women undergoing frozenthawed single blastocyst transfers in Department of Reproductive Medicine,Women and Children's Hospital,School of Medicine,Xiamen University from January 2012 to July 2019,the clinical data were analyzed retrospectively.The quality of the blastocysts were grouped into four categories:excellent,good,average and poor quality,with 20 cases,73 cases,231 cases and 55 cases,respectively,and the birthweight of singleton was measured.Multiple linear regression analysis was performed to investigate the relationship between embryo quality and neonatal birthweight.Results The mean birthweight of low quality blastocysts were 114.73 g less than the high quality blastocysts,but the difference was not statistically significant(95%CI:-233.276--53.652,P>0.05).The women BMI had an impact on gestational age,and the difference was statistically significant(95%CI:-0.206--0.055,P<0.05).With a smaller BMI the risk of premature birth was greater.Conclusion The single blastocyst transplantation of diferent quality in frozen-embryo transfer cycles had no fect on the neonatal birthweight.
