腹腔镜下胃袖状切除术治疗肥胖症患者减重效果及生存质量的影响

Effects of laparoscopic sleeve gastrectomy on weight loss and quality of life in obese patients

目的探讨腹腔镜下胃袖状切除术(laparoscopic sleeve gastrectomy,LSG)对肥胖症患者减重效果和生存质量(health related quality of life,HRQOL)的影响。方法回顾性收集91例在我院行LSG的肥胖患者的临床基本资料和术后6、12个月随访数据,采用中文版的SF-36量表分析患者术后不同时间生理和心理健康改善情况,评估LSG的减重和生活质量改善效果。结果与术前相比,LSG术后患者体质量、腹围、腰围、臀围及体质指数(body mass index,BMI)均有下降趋。术后12个月体质量较术前明显下降(P<0.05),BMI在术后6个月和12个月与术前相比均有显著降低(P<0.05)。术后6个月和12个月多余体质量减少百分比(percent of excessive weight loss,%EWL)分别为(63.06±22.99)%和(77.66±23.63)%。术后生理机能、生理职能、躯体疼痛、一般健康状况、精力和社会功能等6个维度得分在术后随访时间均较术前明显改善(P<0.01)。生理健康总评及心理健康总评得分较术前有所上升。结论肥胖症患者LSG术后近期减重效果显著,生存质量随时间推移逐渐改善,提示LSG具有良好的控制体重和改善生存质量的作用。

Objective To assess the efficiency of laparoscopic sleeve gastrectomy(LSG)on weight loss and quality of life(QOL)in obese patients.Methods The clinical data and post⁃operative 6⁃and 12⁃month follow⁃up materials of 91 obese patients who underwent LSG in our hospital were collected retrospectively.The SF⁃36 questionnaire including physical and psychological aspects of life were used to determine post⁃operative improve⁃ment of QOL.One⁃way ANOVA was applied to compare the differences between groups and Tukey′s method was used for multiple comparison test.Results Body weight,abdominal,waist,and hip circumference and body mass index(BMI)were all decreased after operation.Body weight 12 months after operation and BMI 6 and 12 months after operation were significantly decreased while the percent of excess weight loss(%EWL)was(63.06±22.99)%and(77.66±23.63)%respectively 6,and 12 months after operation.The score of physical functioning,role physical,bodily pain,general health,vitality,and social functioning was markedly elevated 6,and 12 months after operation.The total physical and psychological condition were improved after LSG.Conclusion Ex⁃cellent short⁃term weight loss and improved QOL are observed after LSG in obese patients.
