羟苯磺酸钙联合骨化三醇对糖尿病肾病血液透析患者肾功能和微炎症状态的影响 被引量:5

Effects of calcium dobesilate combined with calcitriol on renal function and micro-inflammatory state in diabetic kidney disease patients with hemodialysis

摘要 目的探讨羟苯磺酸钙联合骨化三醇治疗对糖尿病肾病(DKD)血液透析患者肾功能和微炎症状态的影响。方法选取2021年1月至2022年3月杭州市第九人民医院收治的DKD血液透析患者60例。采用随机数字表法分为观察组与对照组,各30例。对照组患者给予骨化三醇治疗,观察组患者在对照组治疗基础上给予羟苯磺酸钙治疗。两组治疗疗程均6个月。比较两组疗效、治疗前后血清肾功能指标和炎症因子水平。结果观察组患者治疗总有效率高于对照组(93.33%比73.33%,P<0.05)。两组患者治疗后尿素氮(BUN)、血肌酐(Scr)水平均低于治疗前(均P<0.05),而肌酐清除率(Ccr)水平高于治疗前(P<0.05)。观察组患者治疗后BUN、Scr水平均低于对照组(均P<0.05),而Ccr水平高于对照组(P<0.05)。两组患者治疗后血清超敏C反应蛋白(hs-CRP)、TNF-α和IL-6水平均低于治疗前(均P<0.05);观察组患者治疗后血清hs-CRP、TNF-α和IL-6水平均低于对照组(均P<0.05)。结论羟苯磺酸钙联合骨化三醇对DKD血液透析患者疗效良好,可改善患者肾功能,减轻患者微炎症状态。 Objective To investigate the effect of calcium dobesilate combined with calcitriol on renal function and micro-inflammatory state in patients with diabetic kidney disease(DKD)on hemodialysis.Methods Sixty DKD patients with hemodialysis admitted to Hangzhou Ninth People's Hospital from January 2021 to March 2022 were divided into two groups with 30 cases in each group.Patients in study group was given calcitriol and calcium dobesilate for treatment,while patients in the control group received calcitriol only.The treatment course was 6 months for both groups.The therapeutic effects,renal function and inflammatory factor levels in two groups were compared before and after treatment.Results The total effective rate of study group was higher than that of control group(93.33%vs.73.33%,P<0.05).After treatment the BUN and SCR levels were lower,and Ccr was higher than those before treatment in both groups(all P<0.05);while compared to control group the BUN and SCR levels were lower,CCR was higher in study group after treatment(all P<0.05).Compared to those before treatment,the serum hs-CRP,TNF-αand IL-6 levels were decreased after treatment in both groups(all P<0.05);while the levels in study group were lower than those in the control group(all P<0.05).Conclusion Calcium dobesilate combined with calcitriol is effective to improve renal function and reduce micro inflammation status in hemodialysis patients with DKD.
