个案管理护理模式对老年糖尿病患者自我管理效能、依从性以及血糖水平的影响

Effect of Case Management Nursing Model on Self-management Efficacy,Compliance and Blood Glucose Level in Elderly Patients with Diabetes

目的探究个案管理护理模式对老年糖尿病患者自我管理效能、依从性以及血糖水平的临床护理效果。方法选取2020年1月—2021年6月南平市第一医院收治的140例老年2型糖尿病患者作为研究对象,采用随机数表法分为观察组和对照组,每组70例。对照组予以常规护理,观察组在对照组基础上予以个案管理护理。对比分析两组护理前后的自我管理效能、依从性、血糖水平。结果观察组护理后的自我管理效能比对照组高,依从性比对照组好,血糖水平比对照组低,差异有统计学意义(P<0.05)。结论个案管理护理模式对老年糖尿病的护理效果明显,可有效提升患者自我管理效能、依从性,降低血糖水平,有临床护理价值。

Objective To explore the clinical nursing effect of case management and nursing model on selfmanagement efficacy,compliance and blood glucose level of elderly patients with diabetes.Methods 140 elderly patients with type 2 diabetes admitted to the First Hospital of Nanping from January 2020 to June 2021 were selected as the research object and divided into observation group and control group by random number table method,with 70 cases in each group.The control group was given routine nursing,and the observation group was given case management nursing on the basis of the control group.The self-management efficacy,compliance and blood glucose level of the two groups before and after nursing were compared and analyzed.Results The self-management efficacy of the observation group was higher than that of the control group,the compliance was better than that of the control group,and the blood glucose level was lower than that of the control group,the difference was statistically significant(P<0.05).Conclusion The nursing effect of case management and nursing mode on elderly diabetes mellitus is obvious,which can effectively improve the self-management efficiency and compliance of patients,reduce blood glucose level,and has clinical nursing value.
