血液透析滤过联合血液灌流对糖尿病肾衰竭合并难治性心衰患者的影响研究

Effect of Hemodialyfiltration Combined with Hemoperfusion on Patients with Diabetic Renal Failure Complicated with Refractory Heart Failure

目的探究糖尿病肾衰竭合并难治性心衰患者接受血液透析滤过联合血液灌流的治疗效果。方法选取2019年1月—2021年10月盐城市大丰人民医院接收的80例糖尿病肾衰竭合并难治性心衰患者为研究对象,按入院顺序分为两组,给予常规对症治疗的40例患者纳入对照组,基于常规对症治疗给予血液透析滤过联合血液灌流治疗的40例患者纳入观察组,对比两组肾功能指标水平及心功能改善情况。结果治疗后,观察组尿总蛋白(0.63±0.12)g/d、尿微量白蛋白(81.22±7.33)mg/d、血尿素氮(5.08±0.41)mmol/L低于对照组,差异有统计学意义(P<0.05)。治疗后,观察组BNP(653.78±7.57)pg/mL、hs-CRP(9.26±0.78)mg/L低于对照组,差异有统计学意义(t=149.385、13.792,P<0.05)。结论糖尿病肾衰竭合并难治性心衰患者经血液透析滤过联合血液灌流治疗,对患者心肾功能改善作用显著。

Objective To explore the therapeutic effect of hemodialyfiltration combined with hemoperfusion on diabetes patients with renal failure and refractory heart failure.Methods From January 2019 to October 2021,80 patients with diabetes renal failure complicated with refractory heart failure received by Yancheng Dafeng People's Hospital were selected as the research objects.They were divided into two groups according to the order of admission.40 patients who received conventional symptomatic treatment were included in the control group,and 40 patients who received hemodialyfiltration combined with hemoperfusion based on conventional symptomatic treatment were included in the observation group.The levels of renal function indicators and the improvement of cardiac function were compared between the two groups.Results After treatment,the total urine protein(0.63±0.12)g/d,urinary microalbumin(81.22±7.33)mg/d,and blood urea nitrogen(5.08±0.41)mmol/L in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).After treatment,BNP(653.78±7.57)pg/mL and hs-CRP(9.26±0.78)mg/L in the observation group were significantly lower than those in the control group,and the difference was statistically significant(t=149.385,13.792,P<0.05).Conclusion In patients with diabetic renal failure complicated with refractory heart failure,hemodialysis filtration combined with hemoperfusion can significantly improve cardiac and renal function.
