加强正确处理医疗不良事件教育在口腔科住院医师规范化培训中的应用

Strengthen the application of education on correct treatment of medical adverse events in the standardized training of stomatologists

目的在口腔科住院医师规范化培训(简称住培)引入正确处理医疗不良事件教育,充实医患沟通的培训内容,为住院医师应对与不良事件相关的沟通挑战做好准备,在住院医师的综合临床诊治能力得到提升的同时,可以独立面对患者,系统开展口腔科临床诊疗工作,为患者提供更加便捷、有效和满意的医疗服务。方法对住院医师进行三个相对独立的培训过程(不良事件沟通方面的讲座和讨论、报告临床病例和课堂讨论)。培训前后,全部参与研究的住院医师填写内容相同的不良事件量表,对培训前后不良事件量表中的数据进行统计分析,研究开展正确处理医疗不良事件培训的效果。结果通过培训,住院医师在不良事件发生后的沟通和解决问题方面的能力有所提高,但是当不良事件发生并解决后,住院医师没有充分利用团队协作的力量和意识到团队协作的重要意义。结论在口腔科住培中引入正确处理医疗不良事件教育有利于提高住院医师在与患者进行医疗不良事件的沟通方面的理解和信心。

Objective In oral teaching of standardized residency training,to introduce the education on properly handling medical adverse events,enrich the teaching content of doctor-patient communication,prepare the residents for communication challenges associated with adverse events,and promote the residents'comprehensive clinical diagnosis and treatment ability at the same time,so that they can be independent in the face of patients and carry out the oral clinical diagnosis and treatment work.We also aimed to provide patients with more convenient,effective and satisfactory medical services.Methods Three relatively independent teaching sessions were conducted for residents(lectures and discussions on adverse event communication,reporting of clinical cases,and classroom discussions).Before and after the training,all resident physicians participating in the study filled in the adverse event scale with the same content,and conducted statistical analysis of the data in the adverse event scale before and after the training,so as to study the training effect after the teaching of correct treatment of medical adverseevents.Results Through training,residents improved their ability to communicate and solve problems after the occurrence of adverse events.However,after the occurrence and resolution of adverse events,residents did not fully utilize the power of teamwork and realize the significance of teamwork.Conclusion The introduction of education on correct management of medical adverse events in the standardized training of oral residents can improve the understanding and confidence of residents in communicating with patients about medical adverse events.
