摘要
目的:探讨长期睡眠质量对青年冠状动脉粥样硬化性心脏病(冠心病)严重程度的影响。方法:本研究连续纳入2019年1月—2020年1月就诊于首都医科大学附属北京安贞医院青年(18~44岁)冠心病患者为研究对象。根据冠状动脉(冠脉)造影结果及SYNTAX评分进行严重程度分组(≤22组和>22组)。根据匹兹堡睡眠质量指数(PSQI)进行睡眠质量分组(PSQI≤5组和PSQI>5组)。PSQI≤5代表睡眠质量较好,PSQI>5代表睡眠质量较差。使用logistic回归研究PSQI指数与SYNTAX评分之间的关系。结果:最终纳入366例青年冠心病患者(男性占93.17%,中位年龄41岁),所有患者填写的睡眠习惯均持续1年以上。在调整混杂因素后,PSQI>5组的患者复杂冠脉病变风险增加(OR=2.755,95%CI:1.389~5.465,P=0.004)。此外,PSQI分值越高(OR=1.205,95%CI:1.098~1.322,P<0.001),复杂冠脉病变的风险也越高。结论:在青年冠心病患者中,PSQI分数及分级是复杂冠脉病变的独立预测因素。
Objective: We aimed to explore the effect of long-term sleep quality on the severity of coronary artery disease(CAD) in young adults. Methods: Young patients(18-44 years old) with CAD who were admitted to Beijing Anzhen Hospital Affiliated to Capital Medical University from January 2019 to January 2020 were included. Severity was classified according to coronary angiography and SYNTAX score(SS)(SS≤22 and SS>22 groups). Sleep quality was classified according to the Pittsburgh Sleep Quality Index(PSQI)(PSQI≤5 and PSQI>5 groups). PSQI≤5 represented good sleep quality, and PSQI>5 represented poor sleep quality. Binary logistic regression was used to investigate the relationship between sleep quality and severity of coronary artery. Results: A final total of 366 young CAD patients(93.17% male, median age 41 years) were included. After adjusting for confounding factors, patients with long-term poor sleep quality(PSQI>5) had an increased risk of complex coronary lesions(OR=2.755, 95%CI: 1.389-5.465, P=0.004). In addition, the higher the score of PSQI as a continuous variable(OR=1.205, 95%CI: 1.098-1.322, P<0.001), the higher the risk of complex coronary lesions. Conclusion: In young CAD patients, long-term sleep quality is positively associated with the complexity of coronary lesions, and poor sleep quality is an independent predictor of it.
