不孕症患者子宫内膜厚度与血流动力学的相关性分析

Analysis of Correlation Between Endometrial Thickness and Hemodynamics in Infertile Patients

目的分析经阴道超声检查不孕症患者子宫内膜厚度与血流动力学的相关性。方法选取2018年2月—2022年2月于阳江市妇幼保健院妇科门诊就诊的60例月经周期规律的不孕症患者为观察组,选取60例月经周期规律的健康育龄期女性为对照组。两组均在围卵泡期和黄体中期应用经阴道超声检查子宫内膜厚度和子宫动脉和螺旋动脉血流动力学参数[收缩期峰值血流速度(peak systolic blood flow velocity,PSV)、搏动指数(pulsatility index,PI)、阻力指数(resistance index,RI)],比较两组的差异以及子宫内膜厚度和血流动力学参数的相关性。结果在围卵泡期和黄体中期,观察组子宫内膜厚度小于对照组(P<0.05);观察组子宫动脉和螺旋动脉PI值、RI值均显著高于对照组(P<0.05),PSV值均明显低于同期对照组(P<0.05);观察组动脉血流动力学参数水平均与子宫内膜厚度有相关性(P<0.05)。结论不孕症患者子宫内膜厚度与血流动力学水平呈正相关。

Objective To analyze the correlation between endometrial thickness and hemodynamics in patients with infertility measured by transvaginal ultrasound.Methods From February 2018 to February 2022,a total of 60 infertility female patients with regular menstrual cycles of Yangjiang City Maternal and Child Health Care Hospital were selected as the observation group,and 60 healthy women of childbearing age with regular menstrual cycles were selected as the control group.The endometrial thickness and the hemodynamic parameters[peak systolic blood velocity(PSV),pulsation index(PI)and resistance index(RI)]of uterine artery and spiral artery were measured by transvaginal ultrasound in both groups in the perifollicular phase and metaphluteal phase,and the differences between the two groups were compared,as well as the correlation between endometrial thickness and hemodynamic parameters.Results In the perifollicular phase and metaphluteal phase,endometrial thickness in observation group was significantly lower than that in normal control group(P<0.05).The PI and RI values of uterine artery and spiral artery in observation group were significantly higher than those in control group(P<0.05),and the PSV values were significantly lower than those in control group(P<0.05).The level of arterial hemodynamic parameters in the observation group was correlated with endometrial thickness(P<0.05).Conclusion There is a positively correlation between endometrial thickness and hemodynamic level in infertile patients.
